Job Description


SOME (So Others Might Eat) provides material aid and comfort to our vulnerable neighbors in the District of Columbia, helping them to break the cycle of poverty and homelessness through programs and services that save lives, improve lives, and help to transform the lives of individuals and families, their communities, and the systems and structures that affect them. We meet immediate needs with food, clothing, and healthcare and offer the tools one needs to live with hope, dignity, and greater independence.

SOME (So Others Might Eat) is currently seeking a full-time Senior Director of Senior Services to join our Elderly Services/Abused Elderly program, at the Kuehner House location.

Position Description: Oversees all clinical services and programming offered within SOME Senior Services. Provide clinical expertise for program development, advocacy, outreach, and department growth. Guide clinical and non-clinical teams on all treatment decisions to ensure best practices and uphold standards of care. Manage grant funding awarded to clinical programs and assist in a development team with grant proposals. Supervise clinical care teams and oversee daily operations in southeast and northeast locations.

Covid-19 Considerations: In alignment with DC Health, COVID-19 vaccinations, and boosters are mandated for personnel working in SOME\'s healthcare-designated programs, which currently include Medical, Dental, Population Health, Behavioral Health Services (BHS), Exodus House, Jordan House, Kirwan House, Keuhner Place, Leland Place, and Mary Claire House. Requests for a medical or religious exemption will be considered for reasonable accommodation.

Schedule: Monday-Friday 8:00 am-4:30 pm (40 hours a week)

Qualifications:

Required - Bachelor\'s Degree in Human Services related field; 5-7 years of experience providing and overseeing clinical services to seniors and at-risk populations; 1-2 years experience overseeing the development of new programs and evaluating current standard operating procedures to ensure programs are effective and efficient. Previous experience establishing and sustaining partnerships and communicating with local and/or government agencies.

Preferred- Master\'s degree in social work; 8-10 years of experience providing and overseeing clinical services to seniors and at-risk populations; 3-5 years of supervisory experience; substantial experience overseeing the development of new programs and evaluating current standard operating procedures to ensure programs are effective and efficient. Previous experience establishing and sustaining partnerships, and communicating with local and /or government agencies. Proficient knowledge of Medicaid waiver programming, Medicaid eligibility, and billing. Experience monitoring and reporting on grant programs. LPC or LICSW by the District of Columbia.

Program Management:
  • Oversee all clinical functions across all programming for clients within Senior Services
  • Provide clinical expertise in program development.
  • Provide oversight of program operations and adherence to SOME policies and procedures. Work with Senior Director to create, revise, and update clinical policies and systems of care.
  • Evaluate program performance and develop strategies to achieve goals.
  • Manage and ensure appropriate use and spending of grants awarded to Senior Services on a designated timeline.
  • Assist with lease-ups, conduct resident interviews, co-manage internal waitlists, and obtain leasing documentation.
  • Ensure that all clinical staff has appropriate training and resources to meet the need of clients served.
  • Network with internal or external entities for clinical and housing program referrals
  • Review, audit, sign, and release documentation client charts for accuracy, and compliance with regulations.
  • Assist with research and development of grant proposals, including budget, goals, and data tracking tools.
  • Collect and synthesize monthly program statistics and CURT reports.
  • Provide grant monitoring and reporting.
  • Develop, implement. and update digital systems for the storage and management of clinical data.
  • Oversee assignment and monitoring of PERS (Personal Emergency Response) program for the department.
  • Create and maintain monthly billing documents for tracking devices.
  • Attend community and Department of Aging and Community Living (DACL) provider meetings.
  • Manage PPE supply for staff of the Homebound Senior Program.
Clinical Geriatric Care:
  • Manage, supervise, and assist clinical staff and wellness teams in providing care coordination and clinical case consultation across all Senior Services Programs.
  • Supervise all clinical care team staff offering clinical supervision to licensed staff
  • Receive, respond to, and oversee new clinical program referrals, confirm eligibility, and complete caseload assignments to clinical staff.
  • Monitor and manage inventory of medical, incontinence, and nutritional supplies for all clinical programs. Ensure that regular deliveries are conducted based on client needs
  • Provide clinical services to EPD Waiver beneficiaries and facilitate care coordination amongst providers.
  • Perform individual counseling with clients and make home visits, as needed.
  • Work collaboratively with internal and external programs to improve service delivery and/or partnerships.
  • Provide clinical crisis intervention and prevention as needed using critical decision-making skills.
  • Facilitate interdisciplinary team meetings and provide clinical direction on all treatment decisions.
  • Offer interdepartmental training to the SOME medical clinical, behavior health, and single adult housing staff on geriatric care.
  • Provide case consultation to staff from those departments who reach out requesting assistance with senior clients.
Supervisory:
  • Ensure appropriate department staffing levels in order to meet objectives; assess, evaluate, and hire new employees as needed.
  • Contribute to department budget development, and use sound judgment to leverage resources in accomplishing department goals; Administer the budget and comply with SOME financial reporting requirements.
  • Contribute to strategy development and strive to improve department functions and performance; Assist in the development of standards and goals as appropriate.
  • Submit supervisory and administrative paperwork to Human Resources and Finance, as required; Refer to SOME Employee Handbook for policy guidance.
  • Maintain professional documentation and ensure the program is in compliance with SOME CQI standards
  • Ensure compliance with all state, local, and federal laws governing the workplace and all department functions.
  • Schedule staff work hours, as needed, and monitor for time and attendance; Approve time cards and request for leave.
  • Provide direction and guidance to staff members on department operations and procedures
  • Define work requirements, assign tasks to staff members, and review work for quality and accuracy; Coordinate with Human Resources to update job descriptions as needed.
  • Provide regular feedback on performance, monitor individual performance, conduct performance evaluations, and take steps for performance improvement as needed.
  • Provide coaching and training for staff, assist staff with work processes and policies, and encourage opportunities for professional development.
