The Intake Coordinator is responsible for obtaining accurate and complete information related to all referrals. The Intake Coordinator supports and fosters the philosophy, goals, and objectives of the agency. Essential Responsibilities:
Responsible for obtaining accurate patient referral information
Tracks Hospitalized/Nursing Home Patients
Collaborates with therapy supervisor/therapy schedule to track all pre op/post op patients
Reviews information to insure the following are recorded:
Primary physician
Diagnosis and secondary diagnosis
Verbal start of care
Update referral with W-10 form.
MINIMUM REQUIREMENTS:
Experience with data processing systems.
Demonstrates the ability to communicate effectively.
The Intake Coordinator must possess excellent verbal and written communication skills with knowledge of all agency functions and home care industry. As the representative of the agency, the Intake Coordinator must present a professional attitude and appearance at all times with the ability to work effectively with all types of individuals, while maintaining strict confidentiality of client and agency information.
