The Intake Coordinator is responsible for obtaining accurate and complete information related to all referrals. The Intake Coordinator supports and fosters the philosophy, goals, and objectives of the agency. Job Duties: The following description of job responsibilities and standards is intended to reflect the major responsibilities and duties of the job but is not intended to describe minor duties or other responsibilities that may be assigned from time to time. Essential Responsibilities:
- Responsible for obtaining accurate patient referral information
- Tracks Hospitalized/Nursing Home Patients
- Collaborates with therapy supervisor/therapy schedule to track all pre op/post op patients
- Reviews information to insure the following are recorded:
- Primary physician
- Diagnosis and secondary diagnosis
- Verbal start of care
- Update referral with W-10 form.
- Experience with data processing systems.
- Demonstrates the ability to communicate effectively.
