SUMMARY The Patient Centered Medical Home (PCMH) Project Manager will work under the general supervision of the Chief Medical Officer and collaborate with CHC\xe2\x80\x99s Executive Team to design, initiate and facilitate PCMH work flow processes throughout CHC. The PCMH Project Manager will coordinate training activities related to PCMH data collection, patient tracking, policy and procedure development and work flow changes. The PCMH Project Manager will be responsible for being a subject matter expert on NCQA requirements and for the initial Practice Transformation Certification and Annual NCQA reporting.
ESSENTIAL DUTIES AND RESPONSIBILITIES include the following. Other duties may be assigned with or without notice.
Responsible for initial Practice Transformation and NCQA Certification and ongoing Annual NCQA reporting.
Oversees the development and implementation of a PCMH recognition program for CHC.
Attains expert knowledge of NCQA PCMH standards, policies, processes and application requirements. Advises management of changes to continuously improve content and delivery.
Responsible to manage practice-level aspects of planning, development and implementation of PCMH standards for primary care practices.
Develops new policies and procedures necessary for PCMH implementation. Evaluates existing policies for compliance with the PCMH model.
Evaluates current practice work flows, policies and information to provide feedback to clinics on opportunities for improvement. Develops and implements best practices to improve work flow related to PCMH quality incentives and practice transformation.
Develops, oversees and facilitates training programs to align work flow and Health Department culture with the PCMH model. Provides education and support to staff as they implement and sustain measures to improve chronic disease management.
Oversees the collection, integration and evaluation of diverse data related to clinical and operational performance and medical home transformation activities.
Works collaboratively with other staff members in regard the PCMH model. Evaluates the performance of care teams and makes recommendations for changes. Provides input to the care coordination process. Participates in the quality improvement committee to ensure PCMH issues are included in the overall quality plan.
Researches, gathers and reports health related data to meet NCQA requirements. Provides regular program updates and outcomes to CHC leadership.
Supports electronic health records to ensure that providers and clinicians are using the software to its maximum potential and advantage relative to PCMH designation, meaningful use, and UDS measures.
Plans, organizes and/or facilities ongoing meetings, learning collaborative, seminars and workshops on PCMH standards, transformation topics and practice management to support engaged practices as well as educates practice partners.
Maintains and adheres to HIPAA, employee confidentiality, and privileged communications (patient, employee, and corporation).

EDUCATION AND/OR EXPERIENCE Bachelor\xe2\x80\x99s degree in healthcare, business administration, HIT or related field is highly preferred. A minimum of an Associate\xe2\x80\x99s degree is required if individual does not possess a Bachelor\xe2\x80\x99s degree. Master\xe2\x80\x99s degree is a plus. Must be knowledgeable in NCQA PCMH standards and requirements. Must have experience in project management, PCMH, and/or clinical experience.
LANGUAGE SKILLS Ability to read and interpret documents such as safety rules, operating and maintenance instructions, and procedure manuals. Ability to write routine reports and correspondence. Ability to speak effectively before groups of customers or employees of the organization.
MATHEMATICAL SKILLS Ability to calculate figures and amounts such as discounts, interest, commissions, proportions, percentages, area, circumference, and volume. Ability to apply concepts such as fractions, percentages, ratios, and proportions to practical situations. REASONING ABILITY Ability to apply sound judgment in understanding to carry out instructions in written or oral form. Ability to make appropriate job decisions following standard office policies and past precedents. Ability to deal with problems involving several concrete variables in standardized situations.
COMPUTER SKILLS Experience with word processing, spreadsheets, email, and keyboarding required. Working knowledge of Microsoft Office and Google Suites. Previous working experience in electronic health records preferred.
CERTIFICATES, LICENSES, REGISTRATIONS Possession of current, valid, unrestricted California Driver\'s License (Class C) is required.
OTHER REQUIREMENTS Required to pass a criminal history background check and drug screen upon hire. Annual health examination; annual Tuberculosis skin test clearance or chest x-ray; proof of immunity to MMR, Varicella, and Hepatitis B; proof of Tdap vaccine; during current flu season, must provide proof of influenza vaccine or a signed declination form. If declined, a flu mask is mandatory during flu season. Health screening requirements are subject to change based on CDC recommendations.
