The Role


We are seeking a compassionate, highly organized, and self-motivated

Patient Intake & Staffing Coordinator

to join our dynamic team. This dual-role is pivotal to the heart of our operations--managing the intake process for new clients and ensuring our patients are matched with the right caregivers based on needs, personalities, and availability.

Key Responsibilities



Receive, review, and process referrals from hospitals, physicians, and case managers Collect, verify, and input patient information including insurance and clinical documentation Coordinate with clinical and administrative staff to initiate timely services Maintain accurate records within our EMR system Communicate empathetically and professionally with patients, families, and providers Schedule and follow up on initial assessments and readiness for services Ensure regulatory compliance and internal protocol adherence Match caregivers with clients based on clinical fit, schedule, and preferences Proactively manage and adapt caregiver schedules to reflect patient needs Collaborate with HR to maintain optimal staffing levels and support onboarding of new hires Own delegated operational projects and intake initiatives with full accountability

Qualifications



High school diploma required; associate's or bachelor's degree preferred Prior experience in home care or healthcare intake and/or staffing strongly preferred Excellent organizational, multitasking, and communication skills Proficiency with EMR systems and Microsoft Office suite Able to manage confidential information discreetly and responsibly Eagerness to grow within a fast-paced, rapidly expanding agency Strong problem-solving abilities with a solution-focused mindset Bilingual in English and Spanish a strong plus*

Pay: $22.00 - $32.00 per hour
