\xc2\xb7 Facilitate communication between the patient and the interdisciplinary team.

\xc2\xb7 Develop and maintain a therapeutic relationship by providing education and promoting self-advocacy and independence.

\xc2\xb7 Utilize state-of-the-art technology to document skilled services provided.

Qualifications

• Registered Nurse with Valid CT RN license
• One year of RN experience; Homecare experience preferred.
• Valid driver\xe2\x80\x99s license and reliable transportation required.
• Computer proficiency required.
