Job description:
Provide direct patient care, including administering medications and treatments as prescribed. Conduct thorough patient assessments and document vital signs, medical history, and care plans. Collaborate with healthcare professionals to develop and implement individualized care strategies. Assist with procedures such as IV infusion, catheterization, and wound care while adhering to safety protocols. Support patients with activities of daily living, promoting their independence and comfort. Educate patients and families on health management, medication administration, and wellness strategies. Maintain accurate medical records and documentation in compliance with HIPAA regulations. Participate in discharge planning and case management to ensure seamless transitions of care.

Pay: $46.23 - $48.00 per hour
