Landmark Health was created to transform how healthcare is delivered to the most medically vulnerable members in our community. Our medical group provides home-based medical care to chronically ill patients, many of whom are ill-equipped to navigate our overwhelming healthcare system.

At Landmark, our interdisciplinary teams collaboratively manage our complex patient panels. These teams are led by Physicians, Nurse Practitioners, and Physician Assistants, with supporting care provided by RN Nurse Care Managers, Social Workers, Pharmacists, Behavioral Health, and other employed team members.

OBJECTIVE
The Telephonic Market Outreach Specialist is responsible for increasing patient engagement through telephonic outreach to Landmark members to schedule an in-home clinical visit by a Landmark provider.

You\xe2\x80\x99ll enjoy the flexibility to work remotely * from anywhere within the U.S. as you take on some tough challenges.

Primary Responsibilities:

  • Utilize tried and true telephonic methods of patient outreach and engagement strategies to schedule visits for enrollment into the Landmark Health program
  • Utilize excellent telephonic sales skills to effectively communicate Landmark services and be able to
  • overcome members objections
  • Uses critical thinking skills to create telephonic outreach strategies that are designed to maximize patient awareness and registration into Landmark\xe2\x80\x99s program
  • Leverage reporting and dashboards to prioritize telephonic outreach to members
  • In coordination with Local Outreach Team, support telephonic outreach to non-engaged and engaged patients
  • Successfully meet and/or exceed engagement monthly targets

  • Proficient in the use of Microsoft office Word, Excel and PowerPoint

Preferred Qualifications:
  • Bachelor\xe2\x80\x99s Degree
  • 2+ years of related experience in a sales, direct marketing or customer service capacity
  • Experience in healthcare
