Job Description

The Director is responsible for coordinating comprehensive services for individuals with developmental disabilities in a supported living environment. The Director has direct oversight and responsibility for all residential services provided to people LIT serves. The Director works closely with team leaders, companions and direct support professionals to ensure that each individual's person centered plan is implemented. The Director may maintain a caseload, and is responsible for developing and implementing all facets of home life as close to a normal family unit as possible for that case load. The Director works closely with the Board and other Support Team Leaders to ensure Direct Support Professionals receive training and build the necessary skills to successfully support the individuals the agency works for.

Responsibilities related to maintaining a case load:

Actively participate in the person centered planning process, including but not limited to: attending all planning meetings, coordinating the development of training plans, ensuring all necessary documentation is completed, reviewing progress notes and daily notes, reviewing CS logs to relation to plan, monitoring for the implementation of the plan, completing monthly reviews, and communication with service coordinators.

Coordination and oversight of medical appointments, including attending appointments if necessary.

Face to face visits with individuals in their homes, ensuring that each individual's rights are respected and protected, personal plan objectives are being implemented, and issues are resolved in a timely manner.

Monitoring for consumer safety, including ensuring that each home environment is safe, clean, and in good repair.

Coordination and oversight of maintaining current data in the individual's record, including but not limited to: individual profile, emergency contact numbers, protected health information, release and consent forms, daily progress notes, monthly reviews, MARS and physician orders. Ensuring staff compliance with the Health Insurance Portability Accountability Act of 1996 (HIPPA).

Manage incident management, including but not limited to review of all Community Event Reports, making appropriate and required notifications, and oversight of follow up to all incidents.

Maintain positive communication with family members, guardians, staff, and Regional Center personnel about policies, issues, and/or concerns.

Prepare and submit applicable reports and documentation related to individuals supported by LIT.

Provide supervision and leadership to companions and direct support professionals for implementation of the person-centered plan, identifying community resources, and assisting individuals in being active participants in their respective communities.

Participate in hiring and training of key personnel, including coordinating on going training needed to assist staff in fulfilling responsibilities related to consumer directed support.

Develop staff schedules and ensure appropriate staff coverage is maintained within each individual's home; provide coverage when necessary.

Be available and responsive to companions and direct support staff in emergency and crisis situations, including on call duties.

Performs other related work, as assigned by the Board.

Provide 30 days notice of planned termination of employment.

Additional duties related to coordination of Services:

Provide supervision and leadership to team leaders in the performance of their duties, including but not limited to: meeting regularly to discuss case load issues, assisting team leaders with brainstorming, oversight of team leader job duties, review of paperwork compliance, and setting vision and direction. Oversee recruitment, hiring, training, job design, employee evaluation, and disciplinary process for all employees in residential services. Work with team leaders to take an active role in these processes. Coordination of medical services for those receiving residential services with Community R.N. Coordination of training for staff providing residential services/PA. Handle Workers Comp and Unemployment claims. Manage Billing and Payroll Manage all HRC issues and updates. Manage weekly administrative meetings Manage Budgets for new and existing clients. Coordination of supports for those receiving other services. Ensure compliance with DMH, Medicaid Waiver, and HIPPA standards, and any other licensure or regulatory authorities for those receiving services. Manage growth of residential services in such a way that we are meeting budgetary expectations while also maintaining quality services to those we serve residentially. Lead agency through development and implementation of strategic plan and goals. Manage office budget with direction of the Board. Attend/Travel to mtg. at the request of the Board.

Pay: $60,000.00 - $70,000.00 per year
