Job Description


SUMMARY The Case Manager will work with residents and staff to provide clinical case management of services based on need. They will help locate and acquire services for residents within the agency and in the community. EDUCATION

  • Bachelors degree required and possession of a West Virginia social work license is preferred.

  • Have the ability to make decisions consistent with agency policies and general philosophies of child care.
  • Be an acceptable role model for youth in values and lifestyles in keeping with those ascribed by Burlington.
  • Must have a valid state issued drivers license and submit to an intensive background investigation.
DUTIES AND RESPONSIBILITIES The duties and responsibilities of the Case Manager of Burlington United Methodist Family Services shall encompass at least the following:
  • Responsible for documentation of all meetings and contacts.
  • Complete the CAFAS, APS Care Connect and other necessary assessments.
  • Primarily responsible for finding suitable residence for vacant beds.
  • Establish and maintain ongoing guidance/support system for youth and their families.
  • Schedule, coordinate, and document the Treatment Plan Meetings.
  • Coordinate all necessary services for youth in care.
  • Collect and review billing documentation at the end of each month and submit to the billing office.
  • Conduct individual meeting with each resident in assigned unit. Maintain Contact with the parents/guardian, placing personnel, Juvenile Probation Office, Therapist, Psychologist, Spiritual Life Coordinator, and other interested parties.
  • Complete home visitation forms.
  • Complete monthly summaries no later than the tenth of each month and submit them to the appropriate DHHR worker.
  • Complete court summaries as needed. (Attend court hearings and MDT meetings as necessary).
  • Complete all Case Notes and file in youths clinical chart.
  • Responsible for notifying the Department of Health and Human Resources of any incident such as runners, attempted suicide, changes in behavior, etc., concerning the youth in their care.
  • Maintain ongoing monthly contact with the Department of Health and Human Resources.
  • Meet weekly with the Therapist to review all written documentation such as daily narratives, staff logs, medication records, incident reports, etc.
  • Meet with the Clinical Team after an incident to review incident reports and discuss the incident.
  • Can support the mission and vision of Burlington United Methodist Family Services as evidenced by compliance with all organizational policies and procedures.
  • Supports and facilitates positive interaction with others as evidenced by: professional maturity, respect for others, a team-centered approach, maintenance of confidential information, and an appreciation of a multicultural workplace.
  • Exhibits effective communication skills including proper use of agency communication systems.
  • Exhibits sound stewardship by effectively managing information resources to achieve the objectives of Burlington United Methodist Family Services.
  • Required driving position
    • This position is defined as one that will require you to drive a motor vehicle as part of your daily work. This position does allow the employee to drive motor vehicles owned or leased by Burlington United Methodist Services, Inc., and to transport employees and residents when and if necessary.
  • Performs other duties that fall within the parameters of this position.

The employee will be required to lift, stand, sit, and/or walk for extended periods of time. Repetitive use of hands, wrists, and elbows will be required. The employee will be required to possess the strength and ability to demonstrate techniques taught in various required training. This position will work in an office environment, although sometimes this position must work in extreme weather conditions and potentially hazardous situations due to working in a residential environment.
