Status: Non-Exempt

Working Conditions: Works in a typical office setting and from employee\'s home, or other community location as established by flexible work agreement, to complete paperwork, documentation, and other indirect service tasks. Meets clients in home, foster home, residential facility, hospital, or other community locations as needed; drives an automobile on a routine basis in all types of areas; must have reliable transportation. Requires sitting, standing, bending and reaching. May require lifting up to 25 pounds. Requires manual dexterity sufficient to operate standard office machines such as computers, copiers, calculators, the telephone and other office equipment. Requires normal range of hearing and vision. MIS manages their own schedule, ensuring that they are in the office(s) as needed to complete tasks and attends all required meetings.

Position Summary: The Medical Information Specialist (MIS)/Medical Case Manager is responsible for coordinating/monitoring the medical needs of children in foster care in the St. Louis Region who are case managed by the Missouri Alliance Permanency Program (MAPP) Consortium partners. Primary tasks include ensuring that these children receive the required physicals, immunizations, dental, vision, and hearing screenings and timely documentation of these events.

Essential Job Functions

  • Provides supplementary support to case management, focusing on the medical needs of children in foster care in the St. Louis Region.
  • Coordinates, schedules, and monitors compliance with physicals, immunizations, dental, vision, and hearing screenings. Coordinate consults provided by outside specialties such as psychiatry, podiatry, dentistry, etc. Provides information on and referrals to outside resources as needed.
  • Stays up to date on Children\'s Division medical, dental, and informed consent policy and procedures. Ensures any consents needed are obtained through the appropriate channels in a timely manner. Ensures that these events are documented in a timely manner.
  • Collects/gathers medical information on cases, through the state medical database and/or through medical records requests.
  • Communicates with the assigned FCCM case manager (CM) on a regular basis, ensuring that the CM is aware of all appointments, medical needs, and diagnoses. Works collaboratively with the assigned FCCM case manager, family, placement provider, and Family Support Team, as needed.
  • Provide resources regarding transportation to medical appointments as needed. Plans, arranges, and/or assists with last minute medical intervention services as needed. Provides transportation to medical appointments only with the approval of FCCM Supervisor and/or Director.
  • Interacts with community and medical professionals as required including personnel from the offices of the Children\'s Division, Department of Mental Health- Comprehensive Psychiatric Services and Regional Center, Juvenile Court, schools, psychiatry, and other community providers.
Requirements:

Medical Information Specialist (MIS)/Medical Case Manager shall possess a bachelor\'s degree in social work, psychology, counseling, education, criminal justice, or a related human services field; master\'s degree is preferred. Minimum of two (2) years of relevant child welfare experience needed with a bachelor\'s degree or one (1) years\' experience with a master\'s degree. Knowledge and/or experience working with medical providers, such as a medical social worker, preferred.

Skills:

Represents Presbyterian Children\'s Homes and Services (PCHAS) to the professional community. Must have strong verbal and written communication and ability to represent the agency well. Builds and maintains positive community relationships with MAPP and Children\'s Division personnel, placement and treatment providers, schools, and agency partners. Builds and maintains rapport with children, parents, and families; actively managing and resolving conflicts. Excellent time management, organization, planning, and assertive communication skills.

Works a flexible schedule, as approved by the Regional Director, to be available at varying times to meet the needs of the family (potentially outside of normal working hours). Plans, arranges, and/or assists with last minute medical intervention services as needed. Maintains confidentiality and HIPAA standards for all assigned clients and families.

Must be able to operate computer using agency approved software programs, and other office machines such as copier, calculator, telephone, etc. Must have the ability to navigate multiple data entry systems.

Must be able to analyze many variables and choose the most effective course of intervention at any given point in time. Must handle novel and diverse work problems on a daily basis. Personal maturity is an important attribute. Must be able to resolve problems, handle conflict and make effective decisions under pressure. Must have a long attention span in order to listen to people, perceive the real problems and successfully conclude the issues. Must have the ability to work with people from various ethnic groups in a culturally competent manner.

OTHER REQUIREMENTS:

Must hold a valid driver\'s license, have and maintain a risk-free driving record. Must be willing to undergo initial and periodic child abuse/neglect, criminal history, sexual offender, and child care/elder care disqualification list screenings as conducted by the Missouri Department of Health and Senior Services utilizing the Family Care Safety Registry. As requested, must submit fingerprints for review by the Missouri State Highway Patrol and Federal Bureau of Investigation (FBI). Must undergo a pre-employment drug screen and a pre-employment physical with the results showing no evidence of communicable disease. Must be American Red Cross first aid and CPR certified at all times.
