Compensation: $50.00/DOE + Mileage/Gas Agency Description: Caminar is a nonprofit agency with over 50 years of experience providing community-based support services for people with disabilities. Caminar services are designed to enable adults and older adults with mental health, physical and developmental disabilities to live and work in their community in accordance with their ability and desire. Our mission is to improve the quality of life for people with disabilities by providing opportunities to live in the community with dignity and independence. The agency employs over 400 of the most dedicated and professional staff serving the communities of San Mateo, San Francisco, Solano, Butte and Santa Clara counties. Position Description: Under general supervision of the Program Director (PD) the Case Manager provides direct support to clients, as assigned, including but not limited to assessment, individual and group counseling and crisis intervention. The Case Manager also supports and assists individuals in developing independent living skills and rehabilitation goals, provides liaison to other mental health agencies and professionals, and works with family members and others in the client\xe2\x80\x99s support system. Services are provided in-person and in the community, which may include in client homes. Services are scheduled flexibly according to client need. As such weekly hours may fluctuate according to the clients assigned but would not exceed 29 hrs/week. The Olivos program primarily provides services to individuals in San Mateo County but may serve individuals in other Bay Area counties on a case-by-case basis. Essential Duties & Responsibilities:
  • Support the development and attainment of client rehabilitation goals, symptom management, development of a support system, increasing socialization skills, participating in recreational activities, etc.
  • Assist with and teach activities of daily living, such as meal planning and preparation, personal hygiene, and budgeting.
  • Support and assist with client maintenance of personal health, including attainment of and follow through with medical, psychiatric, and dental care.
  • Maintain a client caseload as assigned.
  • Document in a timely manner treatment planning and interventions according to agency and county billing and quality assurance requirements.
  • Coordinate client treatment with all other members of the treatment team, including family members, conservators, discharge planners, and social workers when clients are hospitalized.
  • Drive own or agency vehicle to treatment destinations, as required; documenting and reporting mileage according to agency procedures, so mileage/gas reimbursement can be made in a timely manner; comply with agency vehicle policy at all times.
  • Participate in an emergency on-call rotation system as defined in the \xe2\x80\x9cEmergency On-Call Agreement\xe2\x80\x9d if/as requested.
  • Monitor cleanliness and maintenance of client living environments, providing assistance as necessary in order to meet minimum standards.
  • In team meetings, contribute to the development of individual treatment and behavior plans.
  • Develop and maintain a respectful, caring, tolerant, ethical, and empowering relationship with all individuals served.
  • Provide monthly accounting of client outcomes and service utilization.
  • Perform all job functions in cooperation with the supervisor, other staff on the case management team, and other service providers involved in the treatment effort, including sharing information regarding all important interventions.
  • Participate in staff meetings and attend other meetings, if/as assigned.
  • Attend and participate in regularly scheduled supervision sessions.
  • Participate in promoting a safe, healthy and clean working environment at all times consistent with applicable laws, industry standards and the agency\xe2\x80\x99s own Health & Safety Program.
  • Complete and submit accurate time keeping information.
  • Promote within the agency and with the general public the philosophy and practice of social rehabilitation.

Qualifications, Skills & Abilities:
  • Must be passionate about Caminar\'s mission.
  • Candidates must be registered with the California BBS; licensed supervision hours available.
  • MA in a mental health related field. Absent MA, a minimum two-years of verifiable program experience in the provision of direct services to individuals with severe mental illness is required.
  • Excellent computer skills in a Microsoft Windows environment including email and Internet navigation required.
  • Experience with electronic health record strongly preferred.

  • Knowledge and demonstrated ability to provide services under the following evidence-based models: Psychosocial Rehabilitation, ACT Model, Full Service Partnership, Harm Reduction, Housing First, and Motivational Interviewing preferred.
  • Ability to maintain a high level of confidentiality, a professional demeanor and to represent the organization in a positive manner at all times.
  • Must demonstrate acceptable level of maturity, good judgment, and emotional stability.

Physical, Environmental and Mental Requirements:
  • Physical: Occasionally required to push/pull objects up to 50 lbs, and to lift/carry objects up to 25 lbs. Frequently required to perform moderately difficult manipulative tasks such as typing, writing, etc. Must be able to walk, stand, sit for extended periods.
  • Sensory: Frequently required to read documents, written reports, and plans. Must be able to distinguish normal sounds with some background noise, as in answering the phone, interacting with residents and staff, etc. Must be able to speak clearly and understand/be understood using the English language.
  • Cognitive: Frequently required to understand and relate to specific ideas, several at a time. Must be able to remember multiple tasks/assignments given to self and others over a period of several days.
  • Environmental Conditions: Frequent exposure to varied office and mixed (residential/office) environments. Occasional exposure to toxins and poisonous substances, dust, and loud noises.
  • Equipment: Frequently required to use a computer, phone, and fax machine.

Special Requirements:
  • Must be able to meet and receive a criminal records clearance, as required by Title XXII, other licensing regulations, and Caminar practices.
  • This position requires frequent driving. A valid California driver license, reliable personal vehicle, current personal auto insurance as required by law, and an MVR sufficient to obtain and reasonably maintain insurability under agency auto liability policies are all essential job requirements.
  • Must be able to pass post offer, pre-employment medical and drug tests as required under State Community Care Licensing regulations and/or agency policies.
  • A personal cell phone with reliable service and, if applicable, a data plan to use for business purposes.
  • May be required to obtain and maintain First Aid and CPR certification.
