Position Concept

Responsible for monitoring, coordinating, supervising, and completing data entry for all program activities and MELSS contracts. Serves as the initial contact for community contacts and other stakeholders in regard to program implementation and maintenance. Oversees Housing Case Managers.

Essential Functions
  • Supervises and monitors all contracted activities/services at assigned sites to ensure model integrity and contractual compliance.
  • Interview, hire, train, and directly supervise Housing Case Managers. Responsible for conducting performance evaluations for assigned staff.
  • Conduct new hire orientation, ensuring the new hire completes employment-related documents and benefits enrollment; also prepare or ensure preparation and submission of personnel transaction forms, evaluations, and similar documents, and obtain background screening information on new hires and candidates.
  • Maintain program/department entry into data systems through accurate and timely input of referrals, client and/or staff demographics, billing, attendance, event, and assessment data. Systems may include, but not limited to EHR and Mindshare systems.
  • Locate and compile information and complete reports as required by contracting agencies; compile, sort, and verify accuracy of data; keep records of work completed; maintain follow-up system on reports requiring action on periodic basis.
  • Maintain accurate and complete records according to Eckerd Connects standards and contractual obligations; client and staff record maintenance to include, but not limited to, filing, retrieval, retention, storage, updating, and destroying files as directed by policies and procedures; maintain confidentiality of client and staff records.
  • Oversees the administration of surveys, pre/post assessments, and other evaluation tools as required for tracking outcome measures. Ensures accuracy and timeliness of data collection on internal and external electronic databases.
  • Track program/department expenditures, ensuring purchasing orders or invoices and receipts are processed in timely manner; Report income and accruals as required; as assigned, prepare invoices for program/department leadership travel; maintain petty cash receipts; ensure p-card transactions are completed in accurate and timely fashion.
  • Networks with community resources to promote program concept, coordinate fund raising efforts to support existing programs and for program expansion.
  • Attend and actively participate in local Community Coalition meetings, Program Community Advisory Council meetings and other local related collaborations.
  • Assist Director in designing and maintaining culturally sensitive services through the programs.
  • Adhere to federal and state laws on confidentiality.
  • Transportation of clients and families may be required to ensure full participation in the program as well as overseeing supervised visitation sessions in cases of emergencies.
  • Adhere to safe practices and ensure safe practice by clients in care.
  • Prepare, maintain, and distribute program/department payroll time logs and timesheets, coordinate changes or corrections.
  • As needed, ensure that all paperwork related to staff injuries and/or incidents is completed and submitted in timely fashion.
  • Serve as member of program/department leadership, quality, treatment, and advisory teams; attend meetings and prepare meeting minutes.
Position Specifications

Education: Bachelor\xe2\x80\x99s Degree, from an accredited College or University, in human services (e.g., psychology, social work, sociology or related field) or higher education required. (If providing therapeutic supervised visitation services or providing clinical supervision to unlicensed staff must be a Master\xe2\x80\x99s level licensed clinician).

Experience: Three years full-time experience working with youth and families required. Experience administering a residential or community-based program preferred.

Knowledge/Skills: Supervision, teaching, communication (verbal, written, listening) organizational (prioritizing tasks, maintaining case notes), counseling, child development, documentation, activity planning, motivation, creativity, data collection.

Other: Must meet state and Eckerd Connects criminal background check requirements.

Travel: Must be able to meet requirements for Eckerd Connects Auto Insurance and be able to drive for business purposes. Must have appropriate and valid state driver\xe2\x80\x99s license.

Physical Demands: Ability to spend extended periods of time in standing/walking/sitting positions in outdoor and classroom environments. Ability to actively participate with clients in recreational activities. Ability to lift up to 30 pounds. Must be free of communicable diseases as defined by the appropriate state and to the extent that knowledge is attainable under federal law. Must be capable of traveling and driving within coverage area, often multiple hours per day.

About Our Program

Eckerd Connects\xe2\x80\x99 Family Support Services is partnered with the Brevard County School District and local agencies to provide assistance to children and families that are homeless and/or unemployed. Services include case management, counseling, and parent training services. Family coaches actively assist families in accessing community resources to secure housing, work skills training programs, employment opportunities, transportation, medical services, and other needs. These intensive services are aimed at reducing the length of time families remain homeless. Additionally, case managers working in the school system assist homeless students in maintaining academic performance and participation in extracurricular activities.
