Position Overview:

As a delegate of Managed Care Organizations, Bluestone\xe2\x80\x99s Care Coordination-led and data driven model of care aims to increase medical support, reduce identified gaps in care and improve overall health and safety of our members.

The Program Manager, Care Coordination is a hybrid role responsible for facilitating the development of internal and external reports and the delivery of data as needed to support and monitor Care Coordination\xe2\x80\x99s programmatic performance including key performance indicators. The data collected will support Care Coordination in improving member experience, quality of care and reduce health care costs. This role will also oversee the Care Coordination training, Engagement and Quality teams. The Program Manager, Care Coordination will assist in the development and continuous improvement of Care Coordination programs including process management and maintenance.

Responsibilities:

  • Assist in the interviewing, training and education of Training, Engagement and Quality team members
  • Organize and oversee training in-person at the office for new hires, as well as refreshers for current Engagement and Quality staff as needed
  • Work collaboratively with Clinical Operations and EHR teams to maximize Bluestone workflows and technology
  • Provide work performance supervision and leadership support to the Training, Engagement and Quality teams
  • Engage in coaching, corrective action, and disciplinary action as needed
  • Provide monthly check-ins and annual reviews to direct reports
  • Manage schedules and timecards, to include paid time off and expense reports for direct reports
  • Develop and maintain policies and procedures/job aides for Operations team
  • Develop efficiencies with other Care Coordination managers and staff
  • Remain up to date on health plan changes and regulations pertaining to Care Coordination Operations
  • Provide backup support to team members as needed
  • Coordinate, facilitate, and participate in meetings for Care Coordination and Bluestone as needed
  • Provide ongoing project support as needed
  • Conduct training sessions following health plan audits
  • In collaboration with Care Coordination Managers, maintain and update all Care Coordination department policies and procedures when changes arise
  • Oversee Training, Engagement and Quality team\xe2\x80\x99s day to day operations
  • Develop, organize and maintain Care Coordination\xe2\x80\x99s quality auditing processes including annual health plan audits
  • Ensure policy compliance by Training, Engagement and Quality team members
  • Work collaboratively with Director/Managers in the development of new programs and processes within Bluestone including the implementation, on-going training and educating staff on new processes
  • Coordinate Care Coordination Value Based Care projects as needed
  • System development participation (i.e., training, education, efficiencies, process improvement, audits, etc.)
  • Policy and Procedure development and ensure all compliance metrics are met
  • Attend all staff meetings, ongoing education opportunities and other collaborative meetings
Qualifications:

Education/Certification/Experience
  • Associate\xe2\x80\x99s or Bachelor\xe2\x80\x99s degree in Human Services, Public Health or a Healthcare related field
  • 3+ years\xe2\x80\x99 experience working in a healthcare related field
  • Health care quality or data analysis experience
Knowledge/Skills/Abilities
  • Ability to analyze new processes and develop useful training tools/guidelines for thoseprocesses
  • Ability to think strategically and demonstrate strong problem-solving skills
  • Ability to balance and prioritize multiple responsibilities and deliverables
  • Possesses leadership skills, collaborative abilities and able to take initiative
