The Licensed Practical Nurse (LPN) provides direct patient care and supports nursing staff in delivering safe, high-quality care in accordance with New Mexico state regulations and facility policies. The LPN works closely with RNs, CNAs, residents, and families to ensure positive health outcomes.
Key Responsibilities:
Provide direct nursing care to residents, including administering medications, treatments, and injections as prescribed.
Monitor residents' conditions, perform routine assessments, and report changes to the RN or provider.
Assist with resident admissions, discharges, and care plan updates.
Maintain accurate and timely documentation in the Electronic Medical Record (EMR).
Perform wound care, catheter care, vital signs, and other assigned clinical tasks.
Supervise and support CNAs, ensuring consistent, high-quality care delivery.
Respond promptly to call lights and resident needs.
Follow infection control policies and safety protocols.
Communicate effectively with residents, families, and the interdisciplinary care team.
Participate in care plan meetings and quality improvement activities.
Qualifications:
Active LPN license
in the state of New Mexico (or compact license).
Minimum
1 year of experience
preferred; LTC/SNF experience highly valued.
Current
CPR/BLS certification
.
Strong assessment, communication, and documentation skills.
Ability to work independently and as part of a team in a fast-paced environment.

Work Schedule:
8-hour or 12-hour shifts
available (days, evenings, or nights).
