Licensed Nursing Assistant (LNA) - $2500.00 Sign on Bonus

Suncrest Healthcare Communities (Franklin County Rehab & Villa Rehab) are hiring LNA\xe2\x80\x99s for all three shifts. Come join our dedicated and dynamic team! Hourly rate starting at $17 per hour.

Job Responsibilities:

The LNA assists residents by supporting them with personal hygiene and daily living needs; providing comfort, assisting with transfers and other mobility needs as well as monitoring for changes in care.

Job Duties:

• Conducts job responsibilities in accordance with the FCRC Code of Conduct, policies and procedures, applicable federal and state laws and applicable professional standards.
• Promotes adherence to FCRC\xe2\x80\x99s Code of Business Conduct.
• Provide person-centered direct care to residents on specified shift in accordance with LNA plan of care including but not limited to, bathing, dressing, positioning, toileting, transferring, hair, nail and mouth care, incontinence care.
• Assures resident safety by providing care to attain or maintain their highest physiological, mental and psychosocial wellbeing as determined by individualized plan of care.
• Assure resident safety by following resident specific care plan, transfers, and utilizing gait belts, answering call lights, and addressing resident needs promptly.
• Assure that resident remains free from Abuse or Neglect.
• Takes initiative to engage with residents, identify strengths and abilities and foster independence in provision of care.
• Observes resident for pain, intake and output, skin breakdown, changes in mental status, functional status, overall status and reports any observations promptly to nurse.
• Reports any abuse, neglect, grievance or complaint regarding resident or family to nurse.
• Practice standards of practice and facility policies for infection control, body mechanics, operations of lifts and equipment related to provision of care.
• Offers foods and fluids to residents and assists with dining experience.
• Ensures that weight, reweights, intake and output are completed, tracked and documented.
• Perform rounds on unit ensuring all resident needs are met, rooms are tidy, clean and stocked appropriately and that equipment is clean.
• Works as a team on each shift to assure that all resident needs are met, and other duties are completed.

Skills and Qualifications:

• Graduate of State approved LNA program
• Possess a current State of Vermont license
• Meet physical requirements and pass a Functional Capacity Exam
• Ability to communicate effectively with sensitivity towards patients and their families other staff members and facility visitors
• Must possess willingness to work as team with professional and non-professional personnel and resident families
• Must be able to accept and learn from constructive feedback provided by supervisory staff on unit and within facility
