Position Summary:
The Social Worker is expected to support the mission, vision, and values of Olathe Medical Center, Inc. This position is responsible for consultation and direction of social services to meet the needs of Hospice and/or Home Health patients. He/She will assure that social service/counseling services are integrated and coordinated with other team members. He/She will uphold the philosophy of the Hospice Agency in meeting comfort care needs of the patient and family.
Education:
Graduate Degree - Licensed Masters in Social Work - Required
Experience:
1 - 3 years in Social Work related field, preferably Community-Based Services and/or healthcare setting - Required

Verbal Communication
Organizational
Customer Relations
Clinical documentation: medical records
Licenses/Certifications:
Licensed by the state of Kansas to perform Social Work - LMSW - Required
