Hospice Social Worker
Responsibilities:
- Provide Clinical Social Work to Hospice population.
- Complete initial and ongoing social evaluation of patient and family dynamics
- Participates in the coordination of services with other hospice team members and community resources as needed.
- Maintains and processes all documentation in compliance with agency standards.
- MSW degree in Social Work
- Licensed Social Worker in the state of Massachusetts
- Minimum 1 year of clinical experience; Home Care or Hospice highly preferred
- Valid Driver\'s License, auto insurance and reliable transportation
- Compassion and integrity
