Overview

The Homemaker performs Instrumental Activities of daily living (IADLs) as assigned in an individual\xe2\x80\x99s care plan. Care is generally provided in the home, however, errands, and escort services may also be assigned as designated in an individual\xe2\x80\x99s care plan.
Responsibilities

Comply with all policies and procedures set out by the Hope Nursing Home Care
/ CBHomeCare Employee Manual provided at orientation or any additional
materials provided during the term of employment.
\xef\x82\xb7 Position is responsible for providing services as assigned by the RN Field
Supervisor in accordance with the individual\xe2\x80\x99s approved service plan.
\xef\x82\xb7 The Homemaker is required to report any deviations in an individual\xe2\x80\x99s functional
or medical condition to the appropriate agency personnel.
\xef\x82\xb7 Services must be provided during the assigned schedule for each individual.
\xef\x82\xb7 Deviations from the assignment including, but not limited to the following must be
reported to the agency.
o Deviations from the assigned schedule
o Individual declining specific tasks or requesting unassigned tasks
o Individual expecting to be unavailable for a shift or multiple shifts.
o Hospitalizations, Rehab, or Facility placement.
o Incidents, accidents or other events that are not aligned with the
individual\xe2\x80\x99s plan of care.

\xef\x82\xb7 The Homemaker must be able to demonstrate competence with assigned IADLs
and perform these tasks in line with the plan of care.
o IADLs may include but are not limited to:
\xef\x82\xa7 Cooking
\xef\x82\xa7 Cleaning
\xef\x82\xa7 Shopping
\xef\x82\xa7 Escort
o Companion or Sitter services, as assigned in the care plan

\xef\x82\xb7 If additional services are requested, the Homemaker must notify the RN Field
Supervisor for further instruction.
\xef\x82\xb7 The Homemaker may not negotiate additional tasks, service location, or
schedule changes directly with the individual. The office must be notified for any
alterations, failure to do so may result in termination.
\xef\x82\xb7 Be present for Supervisory Visits to be performed by RN Field Supervisor as
required.
\xef\x82\xb7 Accurately report time and attendance for branch office.
\xef\x82\xb7 Time and attendance must be logged in via timesheets, or EVV, as required by
payer or Hope Nursing Home Care.
\xef\x82\xb7 Required documenting and reporting as assigned by RN Field Supervisor
\xef\x82\xb7 Immediately report concerns of Abuse, Neglect or Exploitation to the office
\xef\x82\xb7 Cell phones may not be used during assignment, unless approved for EVV
services, as instructed
\xef\x82\xb7 Report planned absences to the office as soon as possible so the scheduled
outage may be reassigned to another Homemaker.
\xef\x82\xb7 Be an advocate for individuals needing other community services or a high level
of care.
\xef\x82\xb7 Dress appropriately and professionally.
\xef\x82\xb7 Wear nametag at all times during assignment
\xef\x82\xb7 For infection control purposes, Personal Protective Equipment appropriately
during assignment.
\xef\x82\xb7 Comply with rules and requirements with specific programs, as assigned
\xef\x82\xb7 Other duties, as assigned.
Qualifications

Eighteen (18) years of age or older.
An individual\xe2\x80\x99s BCI check complying with Rhode Island State Law.
Experience in Home and Community Based Services, preferred.
Current Tb Skin Test
Proof of vaccination
Punctuality
Desire to help those at home
Professional demeanor and appearance.
Have reliable transportation and proof of current automobile insurance.
Can demonstrate competence providing assigned tasks
