Overview

The Certified Nursing Assistant provides Activities of Daily Living (ADLs) and Instrumental Activities of Daily Living (IADLs) as assigned in an individual\xe2\x80\x99s care plan. Care is generally provided in the home, however, errands, and/or escort services may also be assigned as designated in an individual\xe2\x80\x99s care plan. The CNA may also provide personal care and other tasks that requires special training and competencies that a Homemaker
cannot provide.
Responsibilities

Work Performed:
\xe2\x97\x8f Comply with all policies and procedures set out by the Hope Nursing Home Care
and the CBHomeCare Employee Manual that is provided at orientation or any
additional materials provided at orientation and during the term of employment.
\xe2\x97\x8f Position is responsible for providing services as assigned by the RN Field
Supervisor in accordance with the individual\xe2\x80\x99s approved service plan.
\xe2\x97\x8f The Certified Nursing Assistant is required to report any deviations in an
individual\xe2\x80\x99s functional or medical condition to the appropriate agency personnel.
\xe2\x97\x8f Services must be provided during the assigned schedule for each individual.
\xe2\x97\x8f Deviations from the assignment including, but not limited to the following must be
reported to the agency.
o Deviations from the assigned schedule.
o Individual declining specific tasks or requesting unassigned tasks.
o Individual expecting to be unavailable for a shift or multiple shifts.
o Hospitalizations, Rehab, or Facility placement.
o Incidents, accidents or other events that are not aligned with the
individual\xe2\x80\x99s plan of care.

\xe2\x97\x8f The Certified Nursing Assistant must be able to demonstrate competence with
assigned ADLs and IADLs or special tasks and perform these tasks in line with
the plan of care.
o Tasks may include, but are not limited to:

\xe2\x97\x8f Bathing (assist, bed bath, tub bath, shower, sponge bath)

\xe2\x96\xaa Dressing / Undressing
\xe2\x96\xaa Mouth care
\xe2\x96\xaa Skin care (non-prescription)
\xe2\x96\xaa Hair care
\xe2\x96\xaa Nail care, cleaning and filing only. Nails may not be cut.
\xe2\x96\xaa Mobility
\xe2\x97\x8f Turn and position
\xe2\x97\x8f Transfer
\xe2\x97\x8f Use of lift
\xe2\x97\x8f Assist with ambulation with or without assistive devices
\xe2\x97\x8f Range of motion
\xe2\x96\xaa Toileting
\xe2\x97\x8f Bedpan/urinal
\xe2\x97\x8f Bowel/bladder training program
\xe2\x97\x8f Perineal/catheter care
\xe2\x97\x8f Apply/remove condom (Texas) catheters
\xe2\x97\x8f Assistance with ostomy care as assigned
\xe2\x97\x8f Incontinence care, including, adult diapers and bed pads
\xe2\x96\xaa Eating
\xe2\x97\x8f Meal preparation according to client\xe2\x80\x99s dietary needs or
restrictions, including encouraging fluids, avoiding food
allergens and disease based dietary restrictions. Clients on
swallowing or choking precautions may have special needs
to include soft, pureed, or liquid diets.
\xe2\x97\x8f Assist, monitor or feed the client by mouth, according to level
of care.

o IADLs may include but are not limited to:

\xe2\x97\x8f Cooking
\xe2\x97\x8f Cleaning
\xe2\x97\x8f Laundry
\xe2\x97\x8f Shopping
\xe2\x97\x8f Errands
\xe2\x97\x8f Escort
o Special Tasks

\xe2\x97\x8f Vital signs, as assigned.
\xe2\x97\x8f Height and weight
\xe2\x97\x8f Application of heat/cold
\xe2\x97\x8f Preventative intervention to prevent decubitus ulcers
\xe2\x97\x8f Apply TED hose, as instructed
\xe2\x97\x8f Deep breathing/cough techniques

o Companion or Sitter services, as assigned in the care plan

o In addition to the above tasks, the CNA may be required to provide other
tasks that are required by client\xe2\x80\x99s needs and level of care (proof of
competence required prior to services rendered).

\xe2\x97\x8f All tasks must be performed with Infection Control in mind. These precautions
may require a mask, gloves, and gown.
\xe2\x97\x8f Handwashing and other appropriate precautions should be done before and after
tasks.
\xe2\x97\x8f Assure that the home is safe from obstacles that may cause falls
\xe2\x97\x8f If additional services are requested, the CNA must notify the RN Field Supervisor
for further instruction.
\xe2\x97\x8f The Certified Nursing Assistant may not negotiate additional tasks, service
location, or schedule changes with the individual. The office must be notified of
any alterations, failure to do so may result in disciplinary action up to and
including in termination.
\xe2\x97\x8f Be present for Supervisory Visits to be performed by RN Field Supervisor as
required.
\xe2\x97\x8f Accurately report time and attendance to the branch office.
\xe2\x97\x8f Time and attendance must be logged in via timesheets, or EVV, as required by
payer or Hope Nursing Home Care.
\xe2\x97\x8f Required documenting and reporting as assigned by RN Field Supervisor
\xe2\x97\x8f Immediately report concerns of Abuse, Neglect or Exploitation to the office.
\xe2\x97\x8f Cell phones may not be used during assignment, unless approved for EVV
services, as instructed.
\xe2\x97\x8f Report planned absences to the office as soon as possible so the scheduled
outage may be reassigned to another CNA.
\xe2\x97\x8f Be an advocate for individuals needing other community services or a higher
level of care.
\xe2\x97\x8f Dress appropriately and professionally.
\xe2\x97\x8f Wear nametag at all times during assignment
\xe2\x97\x8f For infection control purposes, Personal Protective Equipment should be used
appropriately during assignment.
\xe2\x97\x8f Comply with rules and requirements of specific programs, as assigned.
\xe2\x97\x8f Other duties, as assigned.

Qualifications

\xe2\x97\x8f Proof of current CNA/CNA-T license.
\xe2\x97\x8f Eighteen (18) years of age or older.
\xe2\x97\x8f Must have a current, active, qualifying national BCI check complying with Rhode
Island State Law.
\xe2\x97\x8f Experience in Home and Community Based Services, preferred.
\xe2\x97\x8f Current Tb Skin Test or Tuberculin titer test. For positive results, a chest x-ray
within the last 5 years will be required.

\xe2\x97\x8f Proof of vaccinations or antibody titers for immunity
o Measles, Mumps, Rubella (MMR)
o Varicella (Chicken Pox)
o Tetanus, Diphtheria, Pertussis (Tdap)
o COVID-19 (completed initial series)
\xe2\x97\x8f Punctuality.
\xe2\x97\x8f Desire to help those at home.
\xe2\x97\x8f Professional demeanor and appearance.
\xe2\x97\x8f Have reliable transportation complying with all state laws.
\xe2\x97\x8f Can demonstrate competence providing assigned tasks.
