Home Health Registered Nurse Position: Per-diem to Full-time

Location: Las Vegas Valley- Las Vegas and surrounding areas

We are seeking a

detail-oriented and compassionate

Per Diem to Full-TIme RN to join our rapidly growing team. This position will grow into full-time quickly as we are growing rapidly and are looking for nurses who have availability and open to accepting full-time work.

Strongly prefer someone with experience in

wound care

and

OASIS

documentation.

This position is per-diem, however

promotion to full time

is possible for the right candidate.

YOUR ROLE:

Provide skilled nursing visits, including

wound care, medication management, IV therapy, and chronic disease education

Perform comprehensive

OASIS Start of Care assessments

- including admission, transfer, re-certification, resumption of care and discharge OASIS for the home care patient Develop and oversee patient care plans in accordance with

Medicare Conditions of Participation (CoPs),

including participation in the development and periodic revision of the physician's Plan of Treatment and processes change orders as needed

Efficient and thorough documentation utilizing EMR software-

Submit clinical

notes no less than weekly,

and progress notes and other clinical record forms outlining the services rendered as indicated. Work closely with physicians, therapists, and caregivers to optimize patient outcomes,

regularly updating referring physician

and others involved in patient's care plan of any changes in the patient's condition or medication Educate patients and families on disease management, self-care, and post-acute recovery Continually strive to improve his/her nursing care skills by attending in-service education, through formal education, attendance at workshops, conferences

WHAT WE'RE LOOKING FOR:

Active RN license in the state of Nevada, with at least one year of nursing experience.

Wound Care experience

Ability to demonstrate self-confidence and positive attitude toward self and others and maintains commitment and enthusiasm to goal achievement Strong knowledge of

Medicare guidelines, CoPs, and home health regulations

- and the willingness to

learn, adapt, and grow

Team player, who thrives in a

What We're Looking For:

?

Active RN license

in Nevada
?

Experience completing OASIS Start of Care assessments and care plans

? Strong knowledge of

Medicare guidelines, CoPs, and home health regulations

? Ability to work independently while maintaining a

patient-first mindset

? Reliable transportation and willingness to travel within

Las Vegas and surrounding area
