Job Summary:

We are seeking a compassionate and experienced

Per Diem Registered Nurse (RN)

to join our home health care team. You will be working with

Medicaid patients.

Each patient visit is

required to be clocked in for 1 hour

, 2-5 times per week.

In this role, you will provide skilled nursing care to patients in their homes, ensuring high-quality, patient-centered care that promotes wellness, independence, and dignity.

Key Responsibilities:

Perform comprehensive nursing assessments for patients in their homes. Develop and implement individualized care plans in collaboration with patients, families, and physicians. Provide skilled nursing services such as wound care, medication administration, IV therapy, disease management, and patient education. Monitor patient conditions and report changes to the supervising physician and care team. Document all patient care accurately and timely in compliance with regulatory and agency standards. Coordinate with interdisciplinary team members including physical therapists, occupational therapists, social workers, and home health aides. Educate patients and families on managing conditions, medication adherence, and healthy lifestyle choices. Maintain patient confidentiality and comply with HIPAA regulations.

Qualifications:

Current RN license in Nevada with no restrictions Minimum of one (1) year of clinical nursing experience, preferably in home health or a similar setting BLS (Basic Life Support) Certification required Valid driver's license and reliable transportation Familiarity with electronic medical records (EMR) systems Ability to work independently and manage time efficiently

Preferred Qualifications:

Experience with OASIS documentation Knowledge of Medicaid regulations Spanish Bilingual a plus

Schedule:

Flexible per diem hours based on your availability and patient needs Some weekend availability may be required

Physical Demands:

Must be able to travel to patient homes Ability to lift up to 50 lbs Frequent standing, walking, and bending required
Pay: From $50.00 per hour
