Midland Care is hiring for our PACE Intake Coordinator, who is responsible for informing potential participants and their families of the benefit of our PACE services. Enroll participants in the PACE program. Midland Care\xe2\x80\x99s Program of All-inclusive Care for the Elderly (PACE) is a not-for-profit health care program that helps people at risk for needing nursing home care who meet a level of care requirement stay in their own homes and communities. Employment Status: Full-Time, Mon-Fri \xe2\x80\x93 8a-5p Salary staring at $48,000/annually (Pay is based off like experience/requirements) Essential Duties & Responsibilities

  • Complete intakes using all information sources available, assesses potential participant\xe2\x80\x99s eligibility for PACE programming or other Midland Care Services.
  • Respond to all initial inquiries from prospective participants.
  • Effectively describe the PACE program and answer questions about the PACE program.
  • Coordinate and track all initial intake performed.
  • Act as participant advocate and liaison between participant, family and Care Team as it pertains to intake and admission.
  • Assists in other areas of the Midland organization as may be needed.

Qualifications Required: One year of experience working with a frail population (elderly or disabled). Must be able to relate well with seniors and their families to deal with sensitive issues and facilitate problem solving. Must possess valid Kansas Driver\xe2\x80\x99s License, in force automobile liability insurance and maintain a Motor Vehicle Record (MVR) that meets organization\xe2\x80\x99s insurability standard as defined by Midland policy. Preferred: Thorough working knowledge of current community health practices for the frail elderly from direct service experience; intake and assessment experience; experience in marketing programs and advocating for elders; working knowledge of the interdisciplinary model of care management; and experience working with cognitively impaired seniors.
