Position Overview: The Centralized Intake Coordinator will be responsible for recruiting and introducing families to agency programs and services, gather information from a family regarding their needs and strengths in order link them to appropriate services and also share information with internal and external providers about available family support services so that they can make referrals with confidence as needed. Provides culturally competent, family-centered services to families including assessing needs, crisis intervention, advocacy, and referrals and linkages with community resources and agency programs. Assesses clients\xe2\x80\x99 eligibility for agency programs and link clients to resources, supports or other services as appropriate. Utilizes a teamwork approach by working collaboratively with internal program/departmental and administrative staff as well as external funders to support and enroll clients to appropriate programs. Processes and screen applicants and referrals to agency programs within mandated program and contractual guidelines. Enters all client related data into appropriate external and agency databases. Ensure all necessary documentation is timely and accurate. Follow ups with clients or other parties to obtain needed and/or missing information and communicate with clients and/or referral sources the status of the referral. Communicate effectively and respectfully with clients and referral sources when screening program referrals. Assists in publicizing agency programs and educating others about the agency and its services. Develops and maintains a working knowledge of Children\xe2\x80\x99s Friend programs, including program goals, eligibility requirements, current capacity, and enrollment practices. Develops and maintains ongoing outreach strategies to inform families and the community of agency programs, services, and activities offered through Children\xe2\x80\x99s Friend. Proactively ensures data integrity by routinely checking for discrepancies and issues in data collection, data entry and report generation and follow agency measures to make corrections and update data when needed. Develops and maintains a complete understanding of the operating systems and internal and external databases with a focus on maximizing the capacities, efficiencies, and integration of systems. Ability to interact with members of the community representing diverse backgrounds and interests. Miscellaneous duties as assigned. Qualifications:
  • Bachelor\xe2\x80\x99s degree in social work or related field preferred.
  • 1-3 years of experience with at-risk families preferred.
  • 1-3 years of professional experience in administrative support, database management, data analysis and report writing preferred.
  • Fluency in English and another language such as Spanish or Portuguese preferred.
  • Strong organizational skills, computer skills and excellent interpersonal skills when dealing with staff, clients, and visitors, as well as the ability to recognize and respect the need for confidentiality.
  • Possess the ability to perform strong outreach.
