JOB SUMMARY The Director of Care Coordination serves as a key leader in our population health strategy by providing strategic leadership and operational oversight for our care management program across all settings of care. This person will serve a central role in the continued development and evolution of care transformation work within the St Mary\'s system and extend to our community partners with a goal to create a patient-centered vs care-setting-centered approach to care coordination. Central to this role is creating systems that foster smooth and seamless transitions of our patients across settings of care and ensuring that we are proactive, intentional, and appropriately standardized about the setting in which the patient is receiving services. As metrics emerge for measuring population health, it will be the responsibility of the Director of Care Coordination to apply and report these measures. Scope of Service: Primarily Weekdays with overall responsibility for operations 24/7. Will oversee utilization review, and social work services and liaise with the clinical documentation team. JOB REQUIREMENTS EDUCATION
  • Bachelor\xe2\x80\x99s Degree required
  • License in a related clinical field is required (RN, Clinician)
  • Valid Maine Driver\xe2\x80\x99s License required
EXPERIENCE
  • Minimum 5 years experience of demonstrated skills in planning and development of multidisciplinary clinical programs with broad knowledge of inpatient/outpatient, community-based health service delivery systems required
  • Minimum 5 years of leadership/supervisory experience
OTHER SKILLS REQUIRED
  • Advanced computer/data analysis skills required
