We are expanding and seeking a qualified Designated Coordinator to join our leadership team.
Job Summary
The Designated Coordinator ensures that all services meet Minnesota 245D licensing requirements and oversees client programs, documentation, compliance, and staff support. This position is critical to maintaining high-quality services and ensuring that all regulatory standards are met.
Primary Responsibilities
Client Coordination
Develop, update, and maintain CSSPs and CSSP Addendums
Ensure service plans meet 245D and HCBS person-centered planning standards
Complete 30-day, 60-day, and annual progress summaries
Maintain consistent communication regarding client needs and services
Attend client visits, planning meetings, and assessments
Staff Supervision and Support
Train, support, and guide Direct Support Professionals
Provide coaching on documentation, compliance, and client supports
Assist with staff performance reviews and corrective actions
Documentation and Compliance
Conduct weekly audits of EVV entries, progress notes, and service records
Ensure services provided align with each client's CSSP and HCBS requirements
Complete and review incident reports and follow-up actions
Maintain accurate, organized, and compliant client files
Assist with DHS licensing, audits, and internal compliance reviews
Quality Assurance
Identify service gaps and recommend improvements
Monitor progress toward client outcomes and goals
Participate in ongoing quality improvement and internal audits
Qualifications
Applicants must meet
one
of the Minnesota 245D Designated Coordinator requirements:
Bachelor's degree in a human services field
or
Licensed Registered Nurse (RN)
or
At least 3 years of direct experience supporting individuals with disabilities in HCBS/245D programs
Additional requirements:
Strong documentation and writing skills
Reliable transportation
Ability to pass a DHS background study
Strong organizational and leadership skills
Ability to train, support, and correct staff professionally
Preferred qualifications:
Experience with 245D services
Knowledge of person-centered planning requirements
Experience with EVV systems
Prior supervisory experience
Pay: $20.00 - $23.00 per hour
Expected hours: 25.0 per week
Work Location: In person
