Job Overview


WE CARE HOME CARE LLC is seeking a passionate and organized

Designated Coordinator / Qualified Professional (QP)

to join our team in

Mankato, MN

. This full-time, on-site position ensures compliance with

MN Statute 245D

and

CFSS

program standards, oversees service delivery, supports staff, and maintains accurate documentation.

The ideal candidate is detail-oriented, compassionate, and knowledgeable about Minnesota DHS regulations and person-centered practices.

Duties



Develop, implement, and review

Individualized Service Plans (ISPs)

and

CFSS care plans

. Conduct regular

quality assurance audits

and service reviews. Supervise and support

Direct Support Professionals (DSPs)

. Maintain compliance documentation, progress notes, and incident reports. Communicate effectively with case managers, guardians, and interdisciplinary teams. Participate in

licensing reviews, staff training, and quality improvement

initiatives. Promote person-centered, respectful, and compliant service delivery.

Qualifications



Meets

Minnesota Department of Human Services (DHS)

qualifications for

Designated Coordinator

or

Qualified Professional (QP)

under

MN Statute 245D

.

Preferred:



Bachelor's degree in

Human Services, Social Work, Nursing, Psychology

, or a related field. 1-2 years of experience coordinating or providing services to individuals with disabilities or vulnerable adults. Experience using

electronic health records (EHR)

or case management systems. Knowledge of

245D

and

CFSS

program standards and compliance practices. Strong organizational, communication, and leadership skills. Attention to detail and ability to manage multiple priorities.
Job Type: Full-time

Pay: $50,000.00 - $65,000.00 per year
