Minimum Qualifications:
Preference will be shown for a candidates who:

  • have previous experience coordinating client services.
Preferred Qualifications:
  • has education, training or experience in human services or a related field.
  • is willing and able to
within 6 months of hire. * has a clean driving record
Principal Duties
A Case Manager at the Utah State Hospital will
  • provide case management from inpatient admission through 90 days post-discharge to ensure a seamless transition to community resources.
  • support patients in following individualized treatment plans and identifying and achieving case management goals.
  • help coordinate care with other health care providers.
  • facilitate connection with community resources related to discharge and successful transition to the community.
  • ensure patient participation in accessing community resources as much as feasible to help the patient learn about the resources and skills to access resources.
  • assist treatment teams and community liaisons to assess, plan for, and provide needed mental health, health, substance use and related services for patients as needed.
  • facilitate effective collaboration by educating community partners and resource agencies about the Utah State Hospital, process, policy and procedures.
  • Aide in criminal court processes.
  • document patient services with hospital electronic records within Utah State Hospital standards.
  • support patients in trial leave and post discharge settings.
