Summary of Responsibilities:



The CP Case Manager is responsible for the provision of home-based services and case management services to the students and their families and/or treatment parents. Duties include but are not limited to:
  • Crisis intervention
  • Advocacy
  • Assessment
  • Discharge planning
  • Interdisciplinary teamwork
  • Community liaison
  • Adheres to standards and regulations as set forth by the agency, the Joint Commission, OCS licensing, DBH and Medicaid
  • abide by all safety rules and regulations of the agency
  • Carries out all duties according to AK Child & Family policies, the AK Child & Family mission, and the AK Child & Family philosophy of care
  • Participates in a rotating on-call service and is responsible for these duties as outlined in policy when on-call
  • Assumes additional duties as assigned
  • All positions at AK Child & Family may have responsibilities altered during the course of employment due to agency imperatives.

PAY:
  • $19.00 + DOE
  • Pay increase after successful completion of 6-month Intro Period.
  • Annual raises are based on the annual budget and not guaranteed each year.
All employees with AK Child & Family are required to be in compliance with the Centers for Medicare and Medicaid COVID vaccine mandate. The mandate requires that by 2/14/22, staff at all health care facilities must have received their first dose of a 2-shot series (Moderna or Pfizer, currently) or a single dose of a 1-shot vaccine (Johnson and Johnson, currently) with the exception of those that have sought and been granted an exemption to the COVID mandate. Staff must complete this step before they can provide any care, treatment or other services for the facility and/or its patients. By 3/15/22, all staff with the exception of those that have sought and been granted an exemption to the COVID mandate, must complete the primary vaccination series.




Supervision and Coordination of Services:




  • The Clinical Case Management Supervisor supervises Case Managers.
  • The Case Manager coordinates services with Activity Therapists, Treatment Parents, and members of the Treatment Team as appropriate.





Qualifications:



Knowledge, Skills and Abilities:
  • The CP Case Manager must have the ability to relate to and experience in working with emotionally disturbed children/adolescents and their families.
  • Demonstrated ability to complete assessments, treatment planning, parent training, advocacy skills and case management skills are also required.
  • The ability to provide written and oral communication in English, along with demonstrated interpersonal/social skills is essential.
  • Creativity/flexibility must be demonstrated in the delivery of services.
  • Excellent time management, organizational skills, and computer skills (MS Office Suite) are also necessary qualifications.
  • The CM must demonstrate the ability to function as a member of the interdisciplinary treatment team and display an understanding and sensitivity to serving a culturally diverse and/or special needs population.
  • The CM must possess a valid driver\xe2\x80\x99s license and ability to travel within the community.

Education and experience:
  • The Case Manager must hold a Bachelor\'s Degree in psychology, counseling, or social work, or a closely related field as identified by inclusion of significant coursework in psychology, child development and related studies and have one year experience working with emotionally disturbed children/adolescents, preferably including case management skills (preferred)
  • OR
  • Five years\' experience working with emotionally disturbed children/adolescents.
  • Knowledge in psycho educational skill building, cognitive and reality based therapies, sexual abuse and neglect, and family skill building is preferred.





Physical Demands:

  • The Case Manager must be able to drive and transport children in personal and/or AK Child and Family vehicles and must be able to access AK Child and Family and community buildings to attend meetings and agency activities.
  • The Case Manager must be able to respond timely to on-call duties on a rotating basis with the DCP, and other community programs clinical therapists.
  • He/she must be able to complete AK Child and Family paperwork and assessments within a timely manner, use the copy and fax machine, complete reports on the computer and produce legible student records.
