Job Summary

:





The Case Manager provides client-centered, trauma-informed, and culturally responsive services to individuals and families, supporting the mission of Servicios de La Raza. This position exercises independent judgment and discretion in assessing client needs, developing comprehensive care plans, and coordinating services. The Case Manager provides psychoeducation, crisis intervention, and harm reduction support, while maintaining compliance with agency standards and regulatory requirements. By engaging with internal and external partners, maintaining accurate documentation, and participating in community outreach, the Case Manager strengthens equitable access to services and enhances client well-being.



This position will support the Re-Entry program, which is committed to breaking the cycle of incarceration in Colorado. The Re-Entry Services program supports individuals transitioning from prison back into the community with the resources they need to succeed. Through direct services, advocacy, and personalized support, we help build pathways to employment, housing, and stability--offering real opportunities for a second chance. Key

Responsibilities:




Assess clients' individualized needs via a comprehensive multifaceted assessment tool addressing social determinants of health Develop comprehensive case management plans and coordinate care, applying professional judgment to prioritize and tailor services Facilitate client-centered, trauma-informed interventions and harm reduction approaches Provide psychoeducation regarding issues relevant to mental health and substance misuse Develop client safety plans as needed Engage in de-escalation and crisis interventions when required, exercising discretion in response strategies Participate in targeted community outreach and events across diverse settings Provide internal and external resources and referrals to support client care Complete all necessary client and department documentation according to SDLR standards Maintain timely and detailed records of all client interactions in accordance with best practices Uphold confidentiality of all client-protected health information following HIPAA guidelines Engage with external community-serving organizations and establish new partnerships Exercise discretion and independent judgment in decision-making, service prioritization, and problem resolution All members of the SDLR workforce are expected to support the agency's community outreach, civic engagement, and recruitment efforts Support the agency's fundraising objectives, which may include tasks such as active fundraising, grant development, solicitation of donations, agency ambassadorship, and informational outreach, and public speaking Commitment to and experience with equity, cultural relevance, diversity, social justice, and disparity reduction practices with persons of color, agencies of color, and underserved communities Ability to establish and maintain effective working relationships with clients, co-workers, agency partners, and the public Other duties as assigned

Bachelor's degree in psychology, human services, social work, or other related field Relevant years of experience will be considered Bilingual preferred (Spanish)
