Per Diem Case Manager - NJ-SNIP Program

The NJ-SNIP Program at JFS serves individuals living with Serious Mental Illness (SMI) through a multidisciplinary team providing therapy, case management, outreach, and intake services. As a Per Diem Case Manager, you'll support clients by developing service plans, connecting them to essential resources, and delivering outreach and case management services. This is a flexible, part-time role requiring availability during regular business hours (Monday-Friday, 9 AM-5 PM), up to 10 hours per week.

Key Responsibilities:

Provide client-centered case management, outreach, and support services Conduct individual, group, and family interventions Coordinate referrals and advocate for access to essential services based on client choice Connect clients to social, spiritual, and community-based resources Complete documentation in alignment with program and agency standards

Position Requirements:

Bachelor's degree in Social Work, Psychology, Behavioral Health, Counseling, or Psychiatric Rehabilitation At least 1 year of experience in a residential mental health setting Valid NJ Driver's License
