Assist Managed Care Organizations (MCOs) members with Pre-tenancy, Tenancy sustaining, and Move-in services to promote stable and healthy living.
Program Case Manager Goal:
The Housing Supports Program Case Manager is responsible for the overall case management of MCOs members seeking housing security. This role is pivotal in ensuring individuals and their families achieve permanent housing stability and improve their overall quality of life, reducing reliance on emergency or temporary housing solutions.
Reports To:
The Housing Supports Program Case Manager reports to the Housing Supports Program Manager and is responsible for the following:
Advance the mission and philosophy of the Mental Health Association of Monmouth County (MHAMC).
Adhere to MHAMC policies and procedures.
Engagement and Outreach:
Build trust and establish rapport with individuals.
Meet individuals where they are, both physically and emotionally.
Needs Assessment:
Conduct needs assessments to identify barriers to housing and sustainability, such as lack of documentation, credit issues, risk of eviction, employment, or transportation challenges.
Develop an individualized housing stabilization plans to support individuals in obtaining and sustaining their housing.
Linkage to Resources:
Connect individuals with appropriate community resources to address their individual needs including obtaining housing and sustainability.
Connect individuals to services such as employment programs, financial assistance, and healthcare to promote housing stability.
Provide referrals to community resources that address basic needs like food, clothing, and transportation.
Collaborate with coordinated entry systems and other service providers to prioritize individuals for housing opportunities.
Serve as a bridge between the individual and service providers.
Crisis Intervention:
Address urgent needs and provide immediate support in times of crisis.
Develop and implement individualized crisis plans as needed.
Housing Assistance:
Assist individuals in identifying, applying for, and obtaining appropriate and affordable housing.
Work with landlords, housing providers, and community partners to secure housing opportunities.
Provide education on tenant rights and responsibilities, budgeting, and conflict resolution.
Help individuals develop housing sustainability for stable and healthy living.
Provide support throughout the housing search and stabilization process.
Advocacy and Empowerment:
Advocate on behalf of individuals to ensure access to services and fair treatment.
Empower individuals to regain control over their lives and achieve self-sufficiency.
Utilize Housing First principles.
Documentation and Compliance:
Maintain accurate records of individuals' interactions, housing progress, and case outcomes in accordance with organizational and regulatory requirements.
Participate in supervision and agency staff meetings.
Community Representation:
Represent MHAMC at community events and meetings as requested.
Skills Required:
Proficiency in assessing general clinical needs and interpret individuals' charts.
Recognize clinical crises requiring emergency intervention.
Cultural sensitivity and effective engagement with diverse populations.
Networking skills and knowledge of formal and informal community resources.
Effective verbal and written communication skills.
Safe driving practices.
Education Requirements
Associate's degree in social work or a related human services field with 2 years of relevant experience. Additional experience may be considered in lieu of formal education. Lived experience is preferred.
