As a Community Case Manager for Living Innovations, you will be responsible for supporting individuals to have a good life at home and in the community. Case Managers at Living Innovations assist by providing coordination and oversight during the planning and delivery of Maine home and community-based waiver services. The ideal candidate will be able to communicate effectively in writing and verbally with coworkers, family members, guardians, and individuals at all levels of the organization. To ensure quality service delivery, all candidates must be able to manage their time and resources efficiently and effectively. Must have BA and two years experience with the IDD populations to be considered. Responsibilities \xc2\xb7 Facilitating communication between members of the support team (individual, guardian, family, service providers and others as appropriate) \xc2\xb7 Working with the support team to develop a person-centered plan. \xc2\xb7 Linkage to appropriate agencies, community resources and informal supports. \xc2\xb7 Monitoring the individual\xe2\x80\x99s progress toward the achievement of objectives specified in the plan. \xc2\xb7 Periodic review of the individual\xe2\x80\x99s status and needs and implementing changes to the plan as needed. Qualifications \xc2\xb7 Bachelor\'s degree. \xc2\xb7 Two years\xe2\x80\x99 experience with IDD/Autism is required. \xc2\xb7 Valid state issued driver\xe2\x80\x99s license, current vehicle insurance declaration and vehicle registration. \xc2\xb7 Must pass comprehensive background checks per state and company regulations/policies.
