Job Summary: The Case Manager is responsible for assisting with recruiting Healthy Start program participants and providing case management support services to preconception, prenatal and inter-conception women, infants, and children and their families.

Essential duties and Responsibilities:

The Case Manager performs a wide range of duties, including, but not limited to the following:
  • Assist with dissemination of information to community members to recruit program participants.
  • Conduct home visits and assessments, including Healthy Start screening tools, substance use screening, depression screenings, and intimate partner violence screenings. Provides education as appropriate.
  • Develop and monitor service plans with all participants.
  • Use a variety of intervention techniques and resources, including Motivational Interviewing, that support participants in establishing behavior change goals.
  • Track participant information and provides proper follow up to care plan. Updates and coordinates the tracking of infant immunization records and medical appointments for mother and child.
  • Serve as an advocate for the participants. Consistently demonstrates ability to link participants/families with community resources. Acts as a liaison between programs that provide and serve participants to ensure smooth interaction.
  • Maintain case load and see all participants on a regular basis as determined by risk level and per supervisor instruction.
  • Attend and assist in all required classes, meetings, in-service trainings, and community activities.
  • Apply skills and knowledge in child abuse/neglect and domestic violence while interacting with participants.
  • Demonstrate organizational skills in independently managing workload and related duties.
  • Will develop positive trait service plans based on strengths within the participant and her family.
  • Work independently to coordinate appointments with participants and maintain a minimum of home visits and telephone contact.
  • Will provide referral and follow up for any additional supportive services required by participant.
  • Document service coordination activities throughout the continuum of care.
  • Assess service plan in consultation with the participant, make appropriate changes to the service plan to ensure progress toward goals.
  • Maintain ongoing contact with participant and involved significant other to ensure adherence to the service plan.
Minimum Education/Certifications: Bachelor\xe2\x80\x99s degree in Social Work, Psychology, or a related human services field, required. Associates degree with adequate case management experience may be considered in lieu of a bachelor\xe2\x80\x99s degree.

Bi-lingual speaking preferred.

Knowledgeable in The Joint Commission regulations, preferred.

Minimum Work Experience: Minimum of two years of experience in human services working in the field or directly in-patient care as a case manager required.

Working with children and families in Public Health, Outreach, Social Work or related field performing care coordination /care management in a medical setting, preferred, but not required if other relevant experience can be demonstrated.

Other Requirements: Reliable transportation to travel throughout the service area.
