Job Description



Aledade Care Solutions is a new business unit at Aledade charged with designing, testing, and deploying new clinical service lines and business offerings that will augment and extend primary care practices\xe2\x80\x99 clinical capacity, offering selected patients a portfolio of virtual and (eventually) home-based services to improve health at lower costs.


We are searching for an Analyst that will support the Kidney Care Management (KCM) team as they look to scale the program across Aledade\xe2\x80\x99s national physician network and reach >3,500 patients by the end of 2023. The Analyst will work across internal and external teams to manage day-to-day tasks and communication, track and report on key metrics, and help drive practice and patient engagement efforts.


Essential Duties & Responsibilities:

  • Maintain KCM project management tools (e.g. program management tracker, bi-weekly update decks), while looking for opportunities to iterate on existing materials
  • Support Practice Adoption and Performance workstreams by collecting and responding to practice and field team feedback, developing/ iterating on program materials, and scheduling/ facilitating calls with practices as needed
  • Organize and facilitate meetings with internal workstreams and external partners; own calendar coordination, agenda creation, note-taking, and proactive follow-up around next steps
  • Support day-to-day relationship management with vendor partner (VillageHealth) and own task management, follow-ups, meeting scheduling, and agenda creation
  • Perform basic quantitative analysis to support program operations
  • Other duties as assigned


Skills and Abilities:

  • Proactive problem-solver with a willingness to learn, ask questions, and operate in ambiguity
  • Strong communication and facilitation skills
  • Strong organizational skills with an attention to detail
  • Working knowledge of Microsoft Excel/ Google Sheets (i.e., must be comfortable with basic formulas like vlookup, pivot tables, etc.)
  • Mission orientation - Demonstrated interest/ excitement around patient impact, ACS and KCM\xe2\x80\x99s mission


Required Education and Experience:

  • Bachelor\xe2\x80\x99s Degree
  • 1+ years of professional working experience
  • Experience working in healthcare
