POSITION SUMMARY A primary point of contact for families interested in seeking services at Judi\'s House, the Bilingual Grief Care Coordinator provides streamlined communication and resources to all families throughout their time at Judi\'s House. With a clear understanding of the purpose and meaning of Judi\'s House programming, the Grief Care Coordinator is called upon to recognize a family\'s needs, conduct assessments, and utilize community resources. They are knowledgeable and familiar with local community agencies and resources and responsible for building strong relationships with community resources. The position has two main components:

  • Clinical Service Coordination
  • Program Administrative Assistance
REQUIRED EXPERIENCE, KNOWLEDGE, SKILLS, AND ABILITIES SOUGHT
  • Verbal and written fluency in Spanish and English.
  • A minimum of two years\' work experience in a helping-related position; preferably with a family focus; and excellent consumer care and attention.
  • Keen attention to detail.
  • Organized, reliable, responsive and planful.
  • Listening and empathy skills.
  • Comfortable speaking to families grieving a recent death loss.
  • Ability to be a member of a cohesive team.
  • An effective communicator and public speaker.
  • Desire to greet grieving children and their families with compassion and caring.
  • Education about human behavior, change and lifespan development.
  • Awareness of self, others, and the environment.
  • Ability to maintain appropriate boundaries with others.
  • Willingness to learn and take on a self-reflective posture.
  • Seek clarification in times of confusion or misunderstanding.
  • Practice healthy work/life integration and individual self-care.
  • Technologically savvy with the ability to quickly learn multiple systems and applications such as Survey Monkey and electronic care records.
EDUCATION OR FORMAL TRAINING
  • Bachelor\'s degree in psychology, social sciences, public health or other related field preferred, associate degree with equivalent experience acceptable.
  • Proficient use of Microsoft Office software including MS Teams, Excel, Word, Outlook, and PowerPoint.
  • Electronic health records experience helpful.
ESSENTIAL DUTIES/RESPONSIBILITIES CLINICAL SERVICE COORDINATION
  • Support In-House and Community-Based Care programming.
  • Answer calls/emails form potential clients and respond to questions about programs with accurate and timely information in both Spanish and English.
  • Conduct phone intakes with bereaved caregivers in Spanish and English.
  • Provide Spanish translation support for verbal and written communications.
  • Enter necessary health information in the electronic health record.
  • Schedule and coordinate family assessments considering staffing resources and capacity.
  • Confirm appointments and gather additional information via phone and email.
  • Prepare, distribute, review and process intake and application materials.
  • Review intake and application materials for completeness and follow up with families regarding missing materials.
  • Assess client paperwork for red flags and communicate concerns to program staff.
  • Assess additional family needs in consultation with the clinical team and create individualized resource recommendations to support needs.
  • Assist families in navigating Judi\'s House intake process and attaining recommended/necessary resources.
  • Update and maintain community resource and referral listings through online research and direct outreach.
  • Maintain clinically accurate and timely records maintenance and management.
  • Address customer services issues/client concerns with compassion and care.
  • Participate in additional events and client activities.
  • Attend outreach events such as resource fairs and fundraisers to increase knowledge and awareness of Judi\'s House programs on childhood grief.
  • Managing relevant data about service provision and workload.
  • Work collaboratively with the clinical team and throughout the organization.
  • Uphold the highest standards of care and ethics.
  • Adhere to and maintain all program policies and procedures and provide consultation, coordination, and support to program staff and Judi\'s House families when needed.
ADMINISTRATIVE ASSISTANCE
  • Answering all incoming calls to Judi\'s House in a friendly, courteous and professional manner.
  • Welcoming and greeting individuals as they arrive.
  • Maintaining organization and cleanliness of workspace.
  • Providing general office support and tasks as assigned.
  • Completing Clinical Team administrative tasks when support is needed.
  • Assisting evaluation team in coordinating and gathering outcome data.
OTHER DUTIES/RESPONSIBILITIES
  • Meeting regularly with Program Team leadership.
  • Occasional evenings/weekends may be necessary.
  • Cultivating the values of JH/JAG throughout the organization.
  • Attending team and organizational meetings, activities, and events as required.
  • Collaborating with Judi\'s House staff and performing job duties to advance the organization\'s mission and vision.
  • Demonstrating ability to work independently and within a team and seek guidance as appropriate.
  • Demonstrating ability to prioritize, attend to, and complete multiple tasks efficiently and effectively.
  • Complying with all organizational policies and procedures.
  • Commitment to JH/JAG mission.
SUPERVISORY DUTIES None PHYSICAL DEMANDS The physical demands described here are representative of those that must be met by an employee to successfully perform the essential functions of this job. While performing the duties of this job, the employee is frequently required to sit, stand, and walk as well as bend, squat, and climb stairs. The employee must occasionally lift and/or move up to 25 pounds. WORK ENVIRONMENT The work environment characteristics described here are representative of those an employee encounters while performing the essential functions of this job. Normal office conditions. The noise level in the work environment is usually quiet. Due to current pandemic restrictions, this position is eligible to work remotely.
