Behavioral Health Solutions (BHS) is now hiring a full-time Behavioral Health Community Liaison in Utah. This role is responsible for coordinating a variety of services for patients in the long-term care (LTC) and skilled nursing facilities (SNF) we support. They will be traveling to all locations in Salt Lake City and surrounding areas providing services as needed and cultivating relationships.

Our team of mental health treatment professionals specialize in providing comprehensive behavioral health services for adults and geriatrics across multiple states. As a provider of behavioral health services, we are seeking skilled professionals of the highest caliber that share our commitment to provide high quality psychiatry and therapy services.

Job Type: Full-time, W2 employment

Responsibilities

• Traveling to long-term care and skilled nursing facilities in Salt Lake City and surrounding areas.
• Care coordination with facility staff, providers, and patients.
• Facilitate a variety of services as needed.
• Educate facility staff on BHS processes and address all questions and concerns.
• Attend Behavior Management meetings with facility staff.

Requirements:

• Current COVID-19 Vaccination and Boosters.
• 3-5 years of experience working in SNF as a Licensed Social Worker, Support Staff or Clinical Care Coordinator.
• Have gone through survey process with previous skilled nursing facilities.
• Knowledge of behavioral/medical terminology, therapy, pharmacology, and related procedures.
• Great attention to detail, are data-driven, and is tech-savvy.
• Strong organizational and time management skills.
• Ability to multi-task and work in a fast-paced environment with changing opportunities.
• Ability to work independently and as part of a team.
• Strong ability to build relationships with the facilities, providers and the supervisory team.
• Excellent written and verbal communication skills.
• Some travel will be required for this role.

Pay: $50,000 - $65,000 annualized salary.
