Introduction information: The Healthcare Transformation Collaboratives program, created under the Illinois Department of Healthcare and Family Services, aims to close gaps in healthcare services and eliminate the barriers to access and inequities that persist in Illinois\xe2\x80\x99 healthcare system. The program seeks to make significant, positive changes to the healthcare system in Illinois and specifically for Medicaid customers. The program encouraged healthcare providers to partner together to further these goals in their communities, creating collaboratives that can leverage their shared resources to create stronger and more innovative strategies for improving access, quality, and equity in the healthcare landscape than they could individually. Pillars Community Health will be part of the West Cook Coalition \xe2\x80\x9cWCC\xe2\x80\x9d. This collaborative will target the health inequities and poor health outcomes that exist in its service area, which includes 18 zip codes in western Cook County, by putting in place a care management model that aims to reach high-risk and rising risk Medicaid customers and uninsured adults. The WCC has come together to support access to care, enhance interventions focused on social determinants of health, and increase health equity and patient outcomes for communities in the West Suburban Cook County. The lack of access to quality healthcare has meant that preventable problems and illnesses have gone unaddressed. The WCC will create plans to solve complex healthcare challenges in holistic ways where there is great need and eliminate barriers to quality care to move toward a healthcare system that truly reaches everyone. The collaboration includes 25 partners who will provide medical, behavioral, and social services. Pillars Community Health will lead the West Cook Coalition\xe2\x80\x99s Behavioral Health Initiatives. The Project Manager will support Pillars Community Health in planning and implementation of multiple projects with several different behavioral health partners with the goals of expanding existing while also launching new community based behavioral health services across the West Suburban Cook County communities. POSITION SUMMARY: This position works closely with Behavioral Health Line of Service and Organizational leadership to effectively manage both internal and external projects and partnerships related to community behavioral health services for the Healthcare Transformation Collaboratives Program. This would include planning, development and implementation of internal projects and ensuring external partners are on track with their project deliverables and milestones. Responsibilities include but are not limited to developing and submitting project plans and deliverables, preparing status reports, establishing effective project communication plans, conducting regular meetings with internal staff and external partners, measuring progress and outcomes, and monitoring project plans to ensure partners are regularly meeting their responsibilities. ESSENTIAL DUTIES AND RESPONSIBILITIES:
  • Develops and manages project plans, inclusive of short and long-term plans to meet targets for milestones and deadlines. Revises plans as appropriate to meet changing needs and requirements.
  • Coordinates with all project partners to assure adherence to project requirements, deadlines, and schedules.
  • Coordinates and facilitates regular meetings with project team members to track progress and identify/resolve any issues.
  • Manages competing priorities and requests from multiple team members and partners and prioritizes appropriately.
  • Ensues project deliverables are completed and adhere to quality standards.
  • Prepares and coordinates the preparation of project status reports by gathering, analyzing, and summarizing relevant information.
  • Establishes effective project communication plans and ensures their execution.
  • Facilitates change requests to ensure that all parties are informed of the impacts on schedule and budget.
  • Establishes and maintains engagement with internal and external project stakeholders.
  • Regularly monitors and measures outcomes to identify successful and unsuccessful project elements.
  • Works collaboratively with colleagues across the organization and coalition to share information, responsibility, and resources in efforts to complete projects.
  • Communicates with Behavioral Health leadership, Organizational Executives, and the Board of Directors as necessary to assure projects remain aligned with strategic organizational goals.

EDUCATION AND EXPERIENCE Bachelor\xe2\x80\x99s degree in Business, Healthcare administration, Social Work, or another related field required. 3-5 years of project management experience required, including 2-3 years project management leadership managing project teams of various sizes. Experience with Agile Project Management (APM) and/or Project Management Professional (PMP) certification preferred. Experience with behavioral health and/or social service systems preferred. Non-profit or healthcare experience preferred. Community health or community mental health experience is highly desired. REQUIRED SKILLS:
  • Ability to lead project teams of various sizes and see them through to completion.
  • Demonstrated understanding of formal project management methodologies.
  • Ability to work effectively with various levels of employees and management; ability to build consensus across diverse groups.
  • Strong writing and communication skills; strong interpersonal skills to motivate and achieve results directly and through others, especially in a diverse work environment.
  • Exceptional organizational and time management skills; great attention to detail and ability to prioritize work tasks for self and others, in a deadline-intensive environment.
  • Ability to delegate effectively.
  • Budget management experience.
OTHER Must maintain a safe driving record and have current valid Illinois Driver\xe2\x80\x99s License. Personal vehicle needed for transportation to/from PCH sites and coalition work-site locations. Mileage for authorized work-related purposes is reimbursable.
