Description and Responsibilities

Adult Community Clinical Services (ACCS) is a Department of Mental Health (DMH) funded service that is designed to deliver evidence-based interventions within a clinically focused model. Individuals served are diagnosed with mental illness or dual diagnosis and reside in the community, either in their own homes, apartments, or in group living environments. Our services focus on helping people transition to a more independent environment while maximizing their natural supports by building skills and achieving personal goals. The ACCS teams are tight knit, supportive and multidisciplinary. Open Sky\xe2\x80\x99s talented clinicians, substance use counselors, nurses, peer staff, housing counselors, outreach counselors, and direct care staff provide quality, compassionate care to the individuals served at ACCS.

The Athol House is a nine-bed residential program located in Athol, MA that provides services for both males and females with varying mental health diagnoses. Services focus on helping people transition to a more independent environment while maximizing their natural supports by building skills and achieving personal goals. Athol House provides individuals with weekly groups such as health and wellness, healthy eating, budgeting, and mindfulness activities. This program is staffed 24/7 and has a strong focus on community relationships.

Residential Counselors support individuals to live as independently as possible, while providing evidence-based treatment and care, within a group living environment. They provide guidance, support, and perform various levels of activities of daily living. Through implementing procedures that ensure the health, safety, and well-being of individuals and general cleanliness and safety of the physical site, counselors ensure individuals are living a healthy, productive, and independent life. Counselors are strong advocates for individuals served, believe in the power of community, and acknowledge the importance of community integration!

In addition, Counselors will:

• Assist in developing individualized, person-centered treatment plans and play a lead role in the daily implementation plans.
• Assist individuals to access and/or provide transportation.

Implement activities that create opportunities for the development of valued roles and personal relationships in the community. * Review and complete all required documentation.

Qualifications

• High School Diploma or equivalent. Alternative education/experience in lieu of formal education will be considered.
• Valid Driver\xe2\x80\x99s License and acceptable driving record.
• Reliability, willingness to learn, and being open to new opportunities.
