Under the direct supervision of the Director of Youth Services, the Youth Crisis Case Manager will play a vital role in providing support and assistance to youth and young adults ages 14-24 experiencing crises and navigating through challenging situations. This position will assume a lead role in crisis intervention efforts for youth served, assessing safety risks and developing crisis intervention plans. Programming will be geared towards promoting self-advocacy in the attainment of their own, client driven life goals. This will be done using the four tasks of International Peer Support: building connection, helping each other understand how we have come to know what we know, redefining help as a co-learning and growing process, and helping each other move toward what is wanted rather than away from what is not wanted. This position is responsible to work off site at a variety of partnering agencies within Baltimore City and assist with the completion of intakes and case management services.
Primary Duties and Responsibilities:
Provide direct case management services to up to 25 participants at any one time.
Conduct comprehensive crisis and needs assessments of all clients seeking services, making referrals to support immediate needs including medical, mental health, or substance abuse treatment.
Develop individualized service plans that guide the provision of high quality, trauma-informed case management services.
Provide crisis intervention support when necessary, including involving local mobile crisis team, law enforcement, Child Protective Services, and other local stakeholders.
Provide crisis counseling and emotional support to clients, utilizing active listening and empathy to establish rapport and build trust.
Provide referrals for community based services including job readiness trainings, individual or group therapeutic services.
Participate in the planning and implementation of group activities as assigned.
Attend weekly multi-disciplinary team meetings in Baltimore City.
Attend scheduled supervisory sessions with the Director of Youth Services to assess program goals, measure success and strategize for ongoing enhancements to ensure we are meeting the needs of our clients.
Maintain monthly contact with active client caseload to monitor service plan goals
Provide housing navigation, intake and coordinated access support for housing match.
Provide direct care services to unaccompanied, transitional and homeless youth ages 14-24 with accessing mental health resource, community resources and coordinated access.
Conducting initial assessments for service delivery.
Develop and implement crisis management & safety plans when indicated.
Conducting home-based, school-based, or field-based service delivery.
Maintain statistics regarding service delivery and other services provided to vulnerable families and individuals as necessary.
Maintain timely documentation in accordance with agency policies in the confidential electronic health record system used by the agency.
Maintain awareness of community resources.
Maintain a strong understanding of trauma-informed principles and approaches, crisis intervention and de-escalation skills, and suicide prevention strategies.
Engage in training to stay abreast of up-to-date trauma informed care practices.
Manage other duties as assigned.

Required Education and Experience:
Master's degree in Social Work, Counseling, Human Services or related field preferred; Bachelor's Degree in the social science or educational fields and at least five years of case management experience required.
A minimum of two years' experience in crisis intervention or suicide prevention related work
Knowledge of and ability to work with persons of diverse backgrounds.
Demonstrated proficiency in crisis intervention techniques and crisis counseling skills.
Proven ability to manage complex client situations centering client safety.
Working knowledge of HIPPA privacy and confidentiality guidelines and regulations.
Required Certificates, Licenses and Registrations:
Training/Certifications in Trauma Informed Care, Crisis Intervention and QPR is preferred for this position

The Crisis Case Manager will need to work in different locations for the agency at different times, varying on the program needs and staff support.
