Position Overview
We are seeking a skilled and compassionate
Wound Care Nurse (RN or LPN)
to join our clinical team. This role is responsible for assessing, managing, and coordinating care for residents with wounds, ostomies, or continence concerns in accordance with clinical standards and facility policies. The Wound Care Nurse will work collaboratively with physicians, nursing staff, and other interdisciplinary team members to promote optimal healing and skin integrity for all residents.
This is a
Monday-Friday, 8-hour/day position with no weekend requirements

Key Responsibilities

Wound & Skin Care Management
Conduct comprehensive
wound assessments and reassessments
for all residents as per facility protocols and clinical guidelines.
Develop, implement, and evaluate
individualized care plans
for residents with wounds, pressure injuries, or ostomies.
Develop and lead skin care programs
to prevent breakdown and promote early detection of skin issues.
Monitor and document
healing progression
, revising care plans based on residents' response to treatment.
Ensure wound care treatments are performed using current best practices and infection control procedures.

Clinical Documentation & Communication
Maintain
accurate and timely documentation
in resident medical records that meets regulatory and facility standards.
Communicate clearly and effectively with physicians, advanced practitioners, and nursing staff regarding wound status, changes, and treatment outcomes.
Participate in
care conferences
and interdisciplinary team meetings to discuss residents' progress and ongoing needs.
Serve as the
clinical resource
to staff for wound prevention and management practices.

Education & Leadership
Provide
in-service training and educational sessions
to nursing and facility staff on wound care, ostomy management, skin integrity, and incontinence prevention.
Mentor and guide nursing staff to ensure adherence to evidence-based wound care practices.
Stay current on new developments, technologies, and best practices in wound and skin care by attending continuing education programs.

Quality Assurance & Compliance
Participate in audits and quality improvement initiatives to ensure compliance with facility and state regulations.
Ensure all wound care supplies and equipment are appropriately maintained and utilized.
Uphold infection prevention and control standards across all clinical activities.

Qualifications & Requirements
Current and active RN or LPN license
in the State of Arizona.
Certification required
in one of the following:
+ Certified Wound, Ostomy, and Continence Nurse (
CWOCN
), or
+ Certified Wound Care Nurse (
CWCN
)
Current CPR certification
obtained through in-person physical assessment.
Must be able to
obtain and maintain a valid Fingerprint Clearance Card
issued by the Arizona Department of Public Safety.
Strong assessment, critical thinking, and documentation skills.
Excellent interpersonal and communication abilities to work effectively with residents, families, and interdisciplinary staff.

Preferred Qualifications
Prior experience in a
skilled nursing facility (SNF)
or
long-term care (LTC)
environment.
Experience with
electronic health record (EHR)
systems and documentation.
