The RN Admission Specialist interviews newly referred patients and or family, reviews the clinical records and completes a physical assessment of the patient. The RN Specialist listens to the concerns and needs of the patient and family and then effectively communicates specifically how Hospice of the Upstate will address the expressed needs/concerns. The hospice eligibility is determined in collaboration with the attending physician and the hospice medical director. If appropriate for admission, hospice informed consents are explained as well as the patient's bill of rights and appropriate signatures are obtained.

Listen to the needs and concerns of the newly referred patient and family
Present/Explain the Hospice of the Upstate program (Home Care vs. Hospice
House)
Collaboratively determine hospice eligibility and appropriate level of care with the attending physician and the hospice medical director
Explain the hospice program and obtain informed consents for enrollment into the HOTU program
Complete admissions per Hospice of the Upstate guidelines, policy and procedure
Participate in the weekend rotation and on-call to provide admission coverage for the weekends
Attend mandatory in-services
Completes all admission documentation by end of business day each day
Provides a detailed report to Hospice Home Care or Hospice House Charge Nurse for hand off communication
DESIRED MINIMUM QUALIFICATIONS
Education and Experience:
Current South Carolina license as a Registered Nurse
Necessary Knowledge, Skills and Abilities:
Understand that referral sources, patients and families are our customers
Demonstrate exceptional customer service to internal and external customers at all times
Understand that when patient/ families or referral source calls HOUS they are seeking help
Practice and accept the goal is to make every effort to help the hospice eligible patient as promptly and efficiently as possible.
Ability to assess and respond to the needs of new hospice referrals in a prompt manner (Response time goal to be determined by department)
Dress in a clean professional manner at all times and represent HOU in a positive respectful way at all times.
Wears clean HOU uniform each day at work
Possesses skills necessary and demonstrates ability to function within a flexible team
Skilled in Nursing Practice, able to cope with family emotional stress and tolerant of individual lifestyles
Computer literate with knowledge of Microsoft Office software
Checks HOU email when working to stay informed
Ability to work successfully with varied groups of people
Maintains professional boundaries within the scope of professional licensure
Demonstrates maturity, initiative and excellent interpersonal and organizational skills
Must be flexible related to duties as priorities can change frequently
Must be empathetic, responsible, able to accept supervision, relate well to patients and families, understand and accept the Hospice philosophy
