Responsibilities
Coordinate patient care plans and ensure effective case management.
Monitor patient progress and adjust care plans as necessary.
Collaborate with healthcare teams to optimize patient outcomes.
Maintain accurate and up-to-date patient records.
Educate patients and families about care plans and health management.
Identify and address barriers to effective care delivery.
Participate in professional development and continuous learning opportunities.
Utilize data to track performance and improve case management processes.
Ensure compliance with healthcare regulations and standards.
Contribute to team meetings and share insights for process improvement.
Develop and maintain strong relationships with patients and healthcare providers.
Assist in the development of case management protocols and procedures.
Experience/Qualifications
Valid RN license in the state of Arizona.
Willingness to work on-site in Tuba City, Arizona.
Bachelor's Degree in Nursing or related field.
Excellent communication and interpersonal skills.
Commitment to continuous learning and professional development.
Minimum of 2 years of experience in case management or a related field.
Strong understanding of case management processes and healthcare regulations.
Ability to work independently and manage multiple cases effectively.
Proficiency in using electronic health records (EHR) systems.
Demonstrated ability to analyze data and improve care processes.
