Job Overview

We are seeking a compassionate and dedicated Licensed Practical Nurse (LPN) to join our healthcare team. The ideal candidate will possess a strong background in patient care and demonstrate proficiency in various nursing skills. As an LPN, you will play a vital role in providing high-quality care to patients across diverse settings, including long-term care, assisted living, and acute care environments. Your expertise will contribute to the well-being of our patients and support their health needs.

Responsibilities

Provide direct patient care, including administering medications, performing wound care, and assisting with daily living activities. Conduct patient assessments and monitor vital signs to ensure optimal health outcomes. Collaborate with healthcare teams to develop and implement individualized care plans. Maintain accurate medical documentation and records in compliance with HIPAA regulations. Assist with IV infusion, catheterization, and other specialized procedures as needed. Support patients with Alzheimer's care, dementia care, and memory care initiatives. Participate in triage and emergency response situations as required. Educate patients and families on health management, medication administration, and treatment plans. Uphold infection control protocols and ensure a safe environment for patients.

Requirements

Valid LPN license in the state of practice. Experience in critical care, long-term care, or assisted living is preferred. Proficiency in EMR/EHR systems such as Epic, Cerner, or eClinicalWorks. Knowledge of medical terminology, ICD coding (ICD-9 & ICD-10), and CPT coding is beneficial. Familiarity with acute pain management techniques and patient assessment skills. Experience working with individuals with developmental disabilities is a plus. Strong communication skills and the ability to work collaboratively within a healthcare team. Basic Life Support (BLS) certification required; additional certifications such as ACLS may be advantageous. Ability to perform venipuncture, specimen collection/processing, and administer injections competently.
