Responsibilities

JOB TITLE: Therapist - Outpatient School Based Tele-therapy

POSITION SUMMARY:

The School Based Tele-therapy Therapists provide clinical and therapeutic support to the students in Wyoming schools through a collaborative contract with Wyoming Behavioral Institute, the Wyoming Department of Education, and each of the counties served. School Based Tele-therapy Therapists will be tasked with providing telehealth services to assigned district schools in order to provide student counseling, advising, and therapeutic services within the school setting. Individual and group counseling are provided on a daily basis to students with an emphasis on skill enhancement and emotional health, while providing supportive care and guidance for students to achieve success and break down barriers to academic performance and assist in resolving social and emotional stressors limiting student performance. School Based Tele-therapy Therapists will collaborate with students, teachers and administrators to implement solutions as indicated through individual, group, or family based therapy. Must be able to produce quality daily chart notes regarding student progress, and write weekly and monthly reports as well as narrative notes, and family support group notes and progress assessments. Will provide trauma-informed treatment services. Will be responsible and accountable for upholding high standards for therapeutic interventions including tracking student progress toward goals and active involvement of the family and other support systems in student success. May make recommendations for students to access other therapeutic services or interventions as needed or as clinically indicated. Other duties as assigned.

BASIC QUALIFICATIONS:

Must be 21 years of age or older and hold a Master\xe2\x80\x99s Degree in Human Services field from accredited college or university. Must hold State of Wyoming Licensure as LCSW or LPC (Provisional licensure may be considered depending upon qualifications). Must have good computer skills and excellent documentation and clinical skills. Must have 1-2 years of experience in therapeutic services in a special education school, a clinical setting such as an inpatient hospital or outpatient clinic working with children or adolescents exhibiting emotional and/or behavioral issues. Must have knowledge and training in treating trauma and providing trauma-informed care as well as the developmental tasks of children, adolescent students, and the ability to apply knowledge in assessment and planning of care. Must have a clean MV record and background and have the ability to travel to multiple school sites and or main WBI hospital daily with own transportation and vehicle insurance, even in inclement weather.

REMOTE EMPLOYMENT OPTION: WBI offers the option of Remote employment for the School Based Tele-therapy Therapists. Those who choose this option must be able to provide evidence of privacy, professionalism, and broadband ability to provide services remotely. Physical inspections of the space may be conducted, virtual supervision is required, and periodic reviews of services will be completed. Employees who choose the option of remote employment will be required to attend in-person orientation and training hours at WBI, which will be coordinated at time of hire.

DUTIES AND RESPONSIBILITIES:

Essential Functions:

  • Complete virtual intake including psychosocial/clinical assessment and other intake paperwork with patient, family, and/or designated guardian.
  • Provide individual and/or group counseling on a daily basis for a variety of conditions/needs, stressing skill enhancement and emotional health.
  • Utilize treatment information from the referring professional(s) in order to obtain more complete information on treatment and to enhance understanding of what their opinion for treatment needs are.
  • Documents assessments on approved WBI forms and in manner consistent with WBI protocols, TJC, Medicare, and Medicaid requirements.
  • Communicate findings in therapist notes in a clear, complete, and timely manner.
  • Assess patient needs for discharge, aftercare planning, and/or referral.
  • Ensure patient/family involvement in treatment planning.
  • Formulate a plan of care based on the multidisciplinary assessments within 3 sessions.
  • Ensure treatment plans include specific measurable goals/objectives/ interventions and defining actions unique to each patient\'s needs and personal growth.
  • Update and revise treatment plans as goals are met or patients\' condition changes, at minimum every 90 days.
  • Ensure documentation on reviews are based on the treatment plan goals/objectives and reflect progress or failure to progress toward goals.
  • Communicate on a regular and timely basis with relevant members of the treatment team and family members as appropriate.
  • Complete all reports, documents, billing sheets, etc. clearly, effectively, and in a timely manner.
Other Functions:

Interventions
  • Facilitate individual, family, and group telehealth therapy sessions effectively to meet the needs of the population served.
  • Evaluate and document effectiveness of interventions and patient responses.
  • Relate to patients in an age-appropriate manner.
  • Attend Individualized Education Plans for student as they come due or as needed; provide feedback at these meetings based on clinical assessments and observations.
  • Coordinate and/or attend team meeting with teachers, parents, outside professional supports, etc. as needed (at minimum every 90 days).
  • Intervenes effectively in unusual and complex patient/family situations.
Discharge Planning
  • Participate in the development of a preliminary discharge plan at admission consistent with patient\'s therapeutic goals.
  • Proficiently coordinating effective discharge processes. Assessing patient readiness for discharge and related teaching needs and referral.
  • Participate in completion of Discharge Instruction Form(s) and documents.
Therapeutic Environment
  • Contribute to the effective functioning of the assigned programs, clinical services, and facility-wide activities and services.
  • Report unsafe conditions to appropriate personnel.
  • Assure patient\'s rights are upheld.
  • Ensures the safe and therapeutic functioning of the telehealth (virtual) interface, including effectively managing connections, minimizing disruption or technical barriers to the extent possible.
  • Create telehealth therapy space to meet privacy standards and ensure safety of patients.
  • Provide telehealth services that are comparable to in-person services.
Provider and/or Supervision of Care
  • Collaborate effectively with physicians and other clinical disciplines in making decisions regarding each patient\'s need for care.
  • Intervene effectively in unusual and complex patient/family situations.
  • Provide accurate clinical safety support in crisis situations and participates in debriefing after crisis situations.
  • Update and coordinate telemedicine therapy services to meet the needs of patients.
  • Communicate effectively with patients, families, health care team members, and community providers regarding patient status and progress.
  • Interact and communicate with community agencies in assessment of and service to patients in a timely manner.
  • Provide liaison and referral services to community agencies and appears in court when requested or subpoenaed.
  • Comply with required standards for medical/legal reporting situations (i.e., child abuse, dependent adult abuse, psychiatric commitment).
  • Represent the facility at legal hearings as requested.
  • Ensures that documentation meets policies, procedures, standards, and laws.
