The role you'll contribute:
This position assists individuals in gaining access to identified medical, social, educational, and other needed services and support. Additionally, this role involves coordinating with various service providers to ensure seamless delivery of services
.

Evaluates and documents patient status, responding appropriately and intervening or referring in a timely manner to ensure optimal patient outcomes.
Demonstrates clinical expertise, competence, and proficiency based on knowledge and skills, exceeding essential job responsibilities per department standards.
Accepts accountability for using evidence to monitor and improve care based on regulatory and practice standards.
Adapts to changing workload, patient needs, technologies, and policies.
Maintains high skill level through participation in CQI, in-services, ongoing education, and attendance at greater than 95% of staff meetings.
Qualifications
The expertise and experience you'll need to succeed:
Bachelor's in behavioral science or related field Required.
Completion or agree to complete AHCA-approved targeted case management training within three months of initially providing services - Preferred
1 years of employment working directly with individuals in a human behavioral health service setting Required
