sition Summary



We are seeking a compassionate and detail-oriented

Case Manager

to join our team. This role focuses on coordinating patient care, supporting treatment plans, and improving health outcomes within a fast-paced healthcare environment.

Key Responsibilities

Assess patient needs and develop individualized care plans. Coordinate with providers across departments, including ICU and PICU. Maintain accurate documentation and medical records. Provide education and resources to patients and families. Support smooth transitions of care and discharge planning. Monitor patient progress and adjust care plans as needed. Advocate for patients to ensure timely access to services.

Qualifications & Skills

Strong knowledge of patient services and case management processes. Familiarity with pediatrics, trauma care, or acute care settings preferred. Proficiency in medical terminology and documentation. Excellent communication and teamwork skills. Ability to thrive in high-pressure environments. Strong organizational and time-management abilities. Compassion, empathy, and professionalism in patient interactions.

Schedule & Work Location

Full-time, 8-hour shift In-person position
