Are you passionate about making a real difference in the lives of others, especially those who've served our country? Mountain CAP of WV is looking for a

dedicated and compassionate Case Manager

to join our Supportive Services for Veteran Families (SSVF) Program team.

In this role, you'll help Veterans and their families find stability, hope, and housing security. You'll work one-on-one with veterans and families, providing strength-based case management in a supportive environment that encourages growth and self-sufficiency.

What You'll Do

Provide individualized, goal-oriented case management to Veterans experiencing homelessness or housing instability. Offer assistance with housing placement, goal setting, and temporary financial support. Build strong relationships with landlords, community partners, and service providers to expand resources for Veterans. Maintain accurate case files, data entry, and reports in accordance with program standards. Collaborate with a caring, mission-driven team dedicated to empowering individuals and strengthening communities.

What We're Looking For

A

bachelor's degree or higher

in social work or a related field. At least

two years of experience

in case management, homeless services, or related human services work is preferred. Excellent communication, organization, and problem-solving skills. Professionalism, empathy, and a commitment to confidentiality. Reliable transportation and the ability to travel within the six-county region. Must pass CIB and APS/CPS background checks.
