JeffCare, a program of Jefferson Parish Human Services Authority, is a Federally Qualified Health Center (FQHC) comprised of two Health Centers. JeffCare\'s goal is integration of primary care and behavioral health services to best serve the needs of the population; and, empowerment of individuals to achieve improved quality of life through the provision of effective services, delivered in an environment of integrity and respect. The screener position is an integral part of the social services team assisting in support of individuals for both primary care and behavioral health and requires an individual with strong interpersonal communication, data management and customer service skills. The incumbent in this position reports to the Health Center Manager (HCM). The incumbent completes all required in-service/trainings in order to maintain organizational compliance as well as adheres to all JPHSA policies and procedures, JeffCare procedures, Accreditation and Recognition requirements.

70% Screens, Registers, and Schedules Individuals Seeking Services from JeffCare:

  • Makes crisis determination and coordinates crisis services between individual, social service supervisors and the HCM;
  • Engages individuals to gather information needed to determine appropriate mental health and substance use (behavioral health services) and/or primary care service needs;
  • Conducts interactive interviews with individuals and obtains information on the presenting problem and financial status;
  • Verifies income on Louisiana Department of Labor per procedure;
  • Verifies third party payor benefits/coverage electronically or telephonically with insurance companies to determine extent of coverage, certification requirements, prior authorization requirements and other policy limitations, deductibles and co insurance amounts, when applicable;
  • Completes prior authorizations as required for individuals to receive initial services;
  • Gathers and documents information received from individuals, including but not limited to demographics, documents required to create a chart, and consent forms;
  • Assists individuals in completing a Discounted Fee Application and computes discounted fees per policy;
  • Informs the individual of all discounted fees or co-pays/deductibles responsible for while receiving services, documents, and obtains individual\'s signature on Assignment of Benefits form;
  • Documents information into the electronic health record concurrently while conducting interviews;
  • Answers inquiries of individuals regarding financial obligations or balances;
  • Schedules/reschedules initial appointments for behavioral health (psychosocials or psychiatric evaluations) and/or primary care services per the scheduling procedures;
  • Arranges for transportation, per procedure, when individuals identify this as a barrier to attending initial appointment; and,
  • Records appointments in EHR.
25% Completes Financial Re-Authorizations As Needed and/or on an Annual Basis:
  • Verifies income on Louisiana Department of Labor per procedure;
  • Verifies continued third party payor benefits/coverage electronically or telephonically with insurance companies to determine extent of coverage, certification requirements, prior authorization requirements and other policy limitations, deductibles and co-insurance amounts, when applicable;
  • Gathers and documents updated information received from individuals, including but not limited to demographics, Assignment of Benefits and consent forms;
  • Assists individuals in completing an updated Discounted Fee Application and computes discounted fees per policy;
  • Informs the individual of all discount fees or co-pays/deductibles individuals are responsible for while receiving services;
  • Completes prior authorizations as required for individuals to continue to receive services;
  • Answers inquiries of individuals regarding financial obligations or balances; and,
  • Documents information into the electronic health record concurrently while conducting interviews.
5% Completes Documentation and Referrals:
  • Obtains aftercare and/or prior treatment records of individuals presenting for screening and registration if not already available;
  • Scans and attaches records to the individual\'s chart in the EHR or saves to the G drive aftercare folder on new and current individuals served;
Physical Requirements
This position is largely a sedentary role; with occasional light lifting up to 10 pounds required. Work activity includes operating a computer, a computer printer, a phone, and a copy machine. Communicates in person, over the computer, and on the phone, and remains in a stationary position 50% of the time in order to accomplish computer work.

Preferred qualifications: Two years of related FQHC, medical office, and/or patient registration experience, experience interviewing individuals to assess needs, and experience working with an electronic health system.

The maximum salary for the Social Service Counselor 3 is $70,117. Qualifications

MINIMUM QUALIFICATIONS:

A baccalaureate degree.

SUBSTITUTIONS:

Six years of full-time work experience in any field may be substituted for the required baccalaureate degree.

Candidates without a baccalaureate degree may combine work experience and college credit to substitute for the baccalaureate degree as follows:

A maximum of 120 semester hours may be combined with experience to substitute for the baccalaureate degree.

30 to 59 semester hours credit will substitute for one year of experience towards the baccalaureate degree.

60 to 89 semester hours credit will substitute for two years of experience towards the baccalaureate degree.

90 to 119 semester hours credit will substitute for three years of experience towards the baccalaureate degree.

120 or more semester hours credit will substitute for four years of experience towards the baccalaureate degree.

College credit earned without obtaining a baccalaureate degree may be substituted for a maximum of four years full-time work experience towards the baccalaureate degree. Candidates with 120 or more semester hours of credit, but without a degree, must also have at least two years of full-time work experience to substitute for the baccalaureate degree.

NECESSARY SPECIAL REQUIREMENT:

Positions that provide addictive disorder counseling services require certification or eligibility for certification as a Counselor in Training (CIT), or a current practice credential as a Certified Addiction Counselor (CAC), Registered Addiction Counselor (RAC), or Licensed Addictive Counselor (LAC) issued by the Addictive Disorder Regulatory Authority (ADRA), or its successor, pursuant to the Addictive Disorders Practice Act, LA RS. 37.3386-3390.6

Positions that provide addictive disorder prevention services require certification or eligibility for certification as a Prevention Specialist in Training (PSIT), or a current practice credential as a Certified Prevention Professional (CPP), Registered Prevention Professional (RPP), Licensed Prevention Professional (LPP) issued by the Addictive Disorder Regulatory Authority (ADRA), or its successor, pursuant to the Addictive Disorders Practice Act, LA RS. 37.3386-3390.6

Examples of Work

Obtains and prepares social histories of clients for information to be reviewed by treatment teams and for use in referrals to other agencies.

Participates in the decision making process of the treatment team including formulating therapeutic activities and recommending a treatment plan.

Manages a caseload by scheduling appointments, evaluating client progress in counseling sessions, and making recommendations to the treatment team.

Coordinates admissions, discharges, and follow-up services within various programs.

Serves as liaison between the client, family, and the agency/institution.

Assists in the procurement of needs such as institutional placements, transportation, financial assistance, medical equipment, shelter, food, clothing, etc.

Assists in determining eligibility for services provided.

Makes recommendations to the criminal justice system or to employers concerning a client\'s progress, including occasional court appearances.
