IMMEDIATE VACANCY: Lahaina Senior Center; Full-Time
NOTE: Employees appointed to temporary positions may also gain membership in the civil service upon satisfactory completion of an initial probation period of at least six months. Once you become a member of the civil service, you become eligible to apply for promotion and transfer opportunities to permanent as well as other temporary positions.

Performs complex work in the development, maintenance and implementation of care plans for the elderly; serves as a member of a case management team; initiates contacts with the elderly and provides information on available community and governmental resources; provides transportation, in-home, and other support services; and performs other related duties as required.

Distinguishing Characteristics:

This class differs from that of the Senior Services Aid I in that the Senior Services Aid II regularly serves as a member of a case management team to establish and update care plans for elderly clientele and monitors plans to ensure that required assistance is furnished; whereas the Senior Services Aid I provides a variety of information, referral, outreach and other support services to the elderly.

This class differs from that of the Senior Services Aid III in that the Senior Services Aid II performs complex work in the coordination, development, maintenance and implementation of care plans for the elderly; whereas the Senior Services Aid III supervises and participates in providing information, referral, outreach and other support services to the elderly.

Examples of Duties

The following are examples of duties and are not necessarily descriptive of any one position in this class. The omission of specific duties statements does not preclude management from assigning such duties if such duties are a logical assignment for the position.

  • Regularly serves as a member of a case management team to establish, update and maintain care plans for the elderly;
  • Assesses information collected in interviews and provided on survey/assessment forms and determines the need for referrals;
  • Refers participants to appropriate government or community agency for required services;
  • Assists participants in obtaining services aiding in the completion of application forms and monitoring the follow through on the application process;
  • May represent clients by speaking in their behalf as an authorized representative at administrative hearings;
  • Follows up to ensure that referrals made were appropriate and met participant\'s needs and requirements;
  • May serve as an interpreter or translator;
  • Locates, interviews and surveys elderly individuals;
  • Receives and disseminates information of interest to participants;
  • Makes regular friendly visitations and reassurance telephone calls;
  • Provides transportation and escort services;
  • Picks up and delivers mail, supplies, groceries, and prescriptions for participants;
  • Assists in the monitoring of medication;
  • Sets up and maintains individual case records and controls;
  • Prepares routine correspondence and reports as required;
  • Gives information over the counter or by telephone regarding program rules and regulations, policies and standards;
  • Assists program specialist in locating and/or advocating for resources to meet the needs of the elderly;
  • May supervise and/or coordinate the activities of program volunteers;
  • Performs other related duties as required.
Minimum Qualification Requirements

Training and Experience: (1) A combination of education and experience substantially equivalent to graduation from high school and two years of experience which shall have involved meeting and dealing with people, one of which shall have involved providing information, referral and outreach assistance to the elderly.

License Requirement: Possession of a valid Hawaii State driver\'s license (Type 3).

Knowledge of: pertinent functions of County, State, and Federal agencies; community resources and their utilization; interviewing techniques; public relations; basic skills in reading, writing and speaking.

Ability to: communicate with clients, relate well to people of all ethnic and socio economic groups; establish and maintain effective relationships with program participants and others; understand and explain written materials.

Health and Physical Condition:

Persons seeking appointment to positions in this class must meet the health and physical condition standards deemed necessary and proper for performance of the duties.

The above examination is being given on an open-competitive basis to establish an eligible list which shall be effective for not more than one year unless extended by the Director of Personnel Services.
