• Hours: Per Diem
• Shift: Day shift
• Location: Diabetes Management \xe2\x80\x93 Fairhaven, MA
• Travel: Driving on average 3 or more times per week required

We are searching for a RN-CDCES to provide diabetes self-management education/training (DSMT) and/or medical nutrition therapy (MNT) to individuals, family members and groups. Manages the care of a distinct patient population through assessment of educational needs, nutritional guidelines, lifestyle interventional goals and counseling. Coordinates patient care in collaboration with other healthcare providers (physicians, pharmacists, case managers, social workers, etc) to develop effective individualized protocols (medications, therapy, nutrition, devices, etc.) that empower patients with the techniques and behavior modification to help control their disease, avoid potential complications and bring about lasting improvements in their quality of life. May provide functional or technical supervision over other employees in same or closely related classifications where scope of supervisory responsibility is limited to occasional direction, guidance, explanation, etc.

Education and/or Experience Requirements:

• Bachelor\xe2\x80\x99s Degree in Nursing is preferred.
• Over one year of related experience required; over three years preferred.

Must be fully vaccinated against seasonal Influenza and the COVID 19 virus and provide required vaccination documentation unless approved for an exemption or deferral by Southcoast prior to starting employment.
